Dear Punong Barangay Hon. Engr. Pedro D. Balite PhD:

I, JESSA MAE P. CELOCIA, 25 years old, single, Filipino, and a resident of 3rd St.
Coca-cola Subd., Iponan, Cagayan de Oro City and Purok-1 Laturan, Libona, Bukidnon
hereby respectfully submit this written application for the issuance of Barangay
Protection Order (BPO), pursuant to Section 8 to 23 of the Republic Act (RA) 9262
or the “Anti-Violence Against Women and Children Act of 2004” in favor of me and
my minor children, JOHAN DAMIEN C. TOLEDO and JEHAN DANIEL C. TOLEDO,
and against my former live-in partner ETHAN DAVE E. TOLEDO, 23 years old, single,
and a resident of Sadalesa Compound, Zone 4, Pasil, Kauswagan, Cagayan de Oro
City.

I have recently filed a criminal case against respondent ETHAN DAVE E. TOLEDO for
Economic, Psychological, and Sexual Abuse under Section 5(e) and 5(i) of RA 9262
last July 19, 2022. The said criminal case is pending resolution before the Office of the
City Prosecutor, Cagayan de Oro City with NPS Docket No. X-06-INV-22G-1507.

After the abovementioned case was filed, there were multiple instances where
respondent had sexually harassed and coerced me both online and personally.
Respondent also keeps on annoying and contacting me directly and even indirectly.
Prior to the filing of case, respondent had also deprived me of custody of my children
and even sent his relatives in taking my minor children in our residence. Attached herein
are the evidences and blotters of the said incident. Thus, I am applying for a Barangay
Protection Order (BPO) to protect me and my children of possible unlawful acts that
respondent might do to me and/or my children as well as to give myself a peace of mind
as I am suffering from PTSD, anxiety, and depression because of the recent acts of
respondent.

It is for these reasons that I respectfully request for the issuance of Barangay Protection
Order (BPO) against the respondent.
