REPORT (PER)
(i) Performance Evaluation Reports (PERs) shall be used as a tool for assessment of employees in a grading
(A,B,C,D) manner to identify the potential of an officer/ official serving in Punjab Emergency Service.
(ii) The Reporting and Countersigning Officers should beauthentic in their opinion about grading of the
employee and grading should be displayed.
(iii) The performance rating should be marked keeping in view the following aspects.
a) Grade-A: means that the employee is exceptionally good in performance of duties as per JDs,
takes lead in implementation of initiatives, self-driven & willing worker and volunteers for
laborious & additional tasks.
b) Grade-B: means that the employee’s performance is satisfactory / Average in terms of job
description.
c) Grade-C: means that the employee’s performance is below average in terms of JDs and needs
improvement.
d) Grade-D: means that the employee’s performance/competency/ commitment/ attitude/ physical
fitness/ financial integrity is unsatisfactory in such a way that makes the employee unfit for
retention in Service.
(iv) The comments on Part-5 (Comments of Reporting Officer) shouldbe mentioned in case of Grade A, C&D
only, clearly indicating the achievements, extraordinary performance, areas to be improved or why unfit to
serve in Punjab Emergency Service.
(v) The Countersigning Officer reserves the right to endorse or disagree the remarks of the Reporting Officer
based on his personal knowledge& experienceabout the concerned officer/ official.Reason for disagreement
with the reporting officer must be mentioned clearly.
(vi) PERs shall be documented and submitted bi-annually i.e. before 15th of January & July every year.
(vii) The overall performance of the official must be in line with the JDs/KPIs mentioned in the bi-
annualPerformance Appraisal Form and monthly feedback recorded in Performance Book.
(viii) The overall Grading of the officers/officials in the Performance Evaluation Report must commensurate with
the Grading in the prime job of the officers/officials.

4. Does simple indexing and filing of correspondence and office records, gather easily ascertained information. A
