“WAIVER”

I Tairan Calvo, parent/legal guardian of Rupert Harold Sotelo, hereby give my son/daughter
permission to attend and participate the 59th annual college days at Ricardo T. Salas (RTS)
Campus on February 10, 2022, as it is one of the activities run by WIT Student Government
Council A.Y 2022 – 2023.

By signing this waiver form, I recognized that the Western Institute of Technology, Office of
the Student Government, and any other representatives and employees of the institution are
not responsible for any unfortunate incident that may arise for reasons beyond their control,
including a COVID-19 transmission case.
