Primary Purpose
The pharmacist will work primarily across wards/clinical pharmacy includes dispensary,
medicines information and rotational weekend on-call services where directed.

Clinical Pharmacy & Medicines • Monitoring medication profiles


Information Service and resolve medication related
issues.
Under the direction of the Clinical • Clinical review including
Pharmacy Team Leader participate in the dosage/medication selection
rotational clinical pharmacy and medicines pending laboratory test results,
information service interactions, adverse reactions,
toxicity, dosage, compliance
formulation and cost.
• Monitoring biochemical,
microbiological and haematological
results where appropriate.
• Medication reconciliation
admission, discharge and transfer
of care.
• Perform clinical interventions when
appropriate, noting recommendations
in patients’ notes, and/or medication
charts.
• Participates in collection of
clinical activity KPI’s
• Monitor prescriptions and
medication charts for legality and
appropriateness.
• Promote cost-effective, evidence-
based prescribing, in conjunction with
the Hospital Medicines List (HML).
• Provide medication counselling to
clients, careers, family and whanau to
improve understanding and adherence
of medication
• Participate in Drug Utilisation Reviews,
Quality Use of Medicines and
Medication Safety initiatives when
required including the reporting,
reviewing of medication incidents
reported via Incident Monitoring
system.
• Conduct literature searches and
provide medication information for
clinicians and clients as requested.
• Involvement in guidelines and
protocols under direction of Clinical
Pharmacy Team Leader or Senior
Pharmacist.
• Undertake research and / or
investigational work which is patient
orientated and designed to improve
patient care and/or departmental
efficiency under direction of senior staff
• Participate in rostered
Medicine’s information service
• Document all medicines
information enquiries in database.

Dispensary & Supply Service • Accurately dispense medications


from prescriptions and medication
charts meeting legal and
departmental requirements.
• Monitor patients’ prescriptions and
medication charts for legality,
availability, and any other medication
related queries.
• Promote adherence to the
Pharmac Hospital Medicines List
(HML) and Special Authority
Criteria
• Dispense and deliver controlled drug
and monitored medicine requisitions to
wards and departments meeting legal
and departmental requirements.
• Be familiar with and refer to appropriate
reference materials when necessary,
for example, the NZF.
• Demonstrate stock control awareness,
and participate in dispensary re-ordering
procedures.

Weekend Rostered Service
• Maintain Pharmacy security at all

• Share extra workloads created by
unforeseen circumstances when
required (including cross cover
between Whakatane and Tauranga
when required).

Pharmacy Leadership team pharmacists.

Person Specification

Essential Desirable
Qualifications • New Zealand Registered Pharmacist, • Having completed or

You are required to meet the Health and Safety at Work Act 2015 requirements as set out in

You are required to maintain a standard of health which will allow for the performance of all
duties and functions of the position. All Te Whatu Ora Hauora a Toi Bay of Plenty sites are
smokefree environments.

Health Practitioners Competence Assurance Act 2003


1. You are required to maintain your current competency based practicing certificate.
2. You must notify your Manager of any changes to scope or conditions on practice
(determined by Regulatory Authority).
3. You must complete the requirements of any competency programme.
4. You must notify your employer of concerns relating to the risk of harm to the public of
another health practitioner practicing below the required standard of competence.
5. Know the provisions of the HPCAA as the governing legislation.
