COVID-19 vaccine policy
As of November 8, 2021, if you are a non-US citizen who is/will be a non-immigrant traveler to the USA (example – F-1 visa holder), you will need
to show proof of being fully vaccinated by an approved COVID-19 vaccine before you travel by air to the United States from another country.

What ways can I show Proof of Funding?

To confirm your place, we need to see proof of funding for your studies through documentation. Depending on
how you pay, this will determine the type of documents you need to provide.

Who will fund your Documentation you will need to provide:

Self An official bank statement (on bank’s letterhead and signed or stamped). The total
amount shown must show at least the amount in the table below for your first
program. The bank statement cannot be older than 6 months.
Family An official bank statement (on bank’s letterhead and stamped or signed) and a
complete Affidavit of Support form (found in this offer packet). Both documents
cannot be older than 6 months, must show at least the amount in the table below for
your first program, and the individual(s) name on both must match.
Official Sponsor A letter of financial guarantee from the sponsor. This must be on official letterhead,
have the student’s name, length of coverage and specify the program and other
costs covered by sponsor.

How much funding do I need to show on a bank statement?

Students studying in the US are required to show proof that they can fund their living and studies in order to
receive an I-20. Find out the type of acceptable funding documents and the amount needed to show to receive

Health Insurance
OSU requires all non-resident international students to have health insurance.

Health Insurance Requirement


• Oregon State University requires that non-resident international students have health insurance which meets the U.S.
Government visa requirements, State of Oregon, and Oregon State University requirements.
• You will automatically receive OSU Student Health Insurance. You will need to pay for this insurance each term.

• You have health insurance from your government sponsor which meets the OSU requirements.
• You submitted a waiver to OSU Student Health Services showing proof of insurance which meets the requirements.

• Payments by check or cash must be made by the third Friday of the term to avoid a “Registration Hold” on your student
account. Students may pay online, through the OSU Student Health Insurance Office or through the OSU Cashier’s Office in Kerr
Administration Building.
• Note that these costs are not included in your INTO OSU invoice.

Family Insurance
Dependants of INTO OSU students (spouse and/or children) are also required by the State of Oregon to have insurance that meets the

Affidavit of Support

Financial Declaration Requirements and Affidavit of Support


If it is determined that you are admissible to Oregon State University, OSU can provide you with an I-20 (F-1) or DS-2019 (J-1) only
after you submit satisfactory evidence that you have adequate funds for your proposed program of study. Acceptable financial
documents must have been issued (by the financial institution) within 6 months of the date the Acceptance Form was submitted
with the Affidavit of Support (below) if necessary. Please refer to the following website for financial amounts required to be shown
at time of application for each program: oregonstate.edu/admissions/international/applying/certification-finances. Be sure to keep
copies of your financial documents as you will need to present them to the US Consular Officer at your visa interview and to the US
Immigration Officers at the Port of Entry.

Parent/Personal sponsor whose name is on financial documents must complete this portion.
Spouses, relatives and other personal sponsors are required to complete the “Affidavit of Support” form when funds
held in their name are provided as financial documentation by an international student at time of application.
The Affidavit of Support helps confirm the legitimacy of financial documentation presented by the student.
