AFFIDAVIT OF LOSS
I, MARK CHRISTIAN L. RODRIQUEZ, of legal age, Filipino citizen, married, and with
residence address at 11 Upper Market Baguio City Philippines, under oath hereby depose and
state THAT:
1. I secured the following documents from the Bureau of Internal Revenue, Baguio City,
Philippines;
a. Books of Account;
b. Certificate of Registration;
c. Ask for Receipt Plate;
2. I placed all the above documents in our house at Upper Market Baguio City,
Philippines which was demolished;
3. When I checked the documents that was retrieved from our demolished house, I found
out that the documents listed above are missing;
4. I exerted diligent effort to locate the said documents, but my effort proved futile,
hence, I am constrained to declare said documents as lost and beyond recovery;
5. I am executing this affidavit to attest to the truth of the foregoing facts especially in
and for all legal intents and purposes it may serve.
