AFFIDAVIT OF KINSHIP
I, MERCIDITA JAVELOSA TABERNILLA, of legal age, Filipino, married, with
residential and postal address at Barangay Mayuwi, City of Tayabas, Province of Quezon,
Philippines, after having been sworn to in accordance with law hereby depose and state that:

1. I am one of the grandchildren of PACIANO JAVELOSA who died sometime in the year
1966;

2. He was survived by his eight (8) children, namely: (1) NORBERTO JAVELOSA; (2)
LOURDES JAVELOSA; (3) BENITO JAVELOSA; (4) JEMMY JAVELOSA; (5)
NORMA JAVELOSA; (6) FELIX JAVELOSA; (7) REYNALDO JAVELOSA; and
(8) JOSELITO JAVELOSA, all of whom are now deceased;

3. That I am one of the closest surviving relative of PACIANO JAVELOSA;

4. I am executing this Affidavit to attest the truth and veracity of the foregoing facts and for
any legal purpose that this may serve.
