 REQUEST FOR APPROVAL

This proposal explains the raffle draw initiative

drawn up by COZA Karu Campus as a means
to encourage members who attended 12DG
2023.

Categories of prizes:
1. COZA TV decoder and 32-inch TV (installation
inclusive)
2. COZA Branded T-shirts
3. COZA branded Jotters and pens
4. 1 month subscription of Pastor's messages
5. Pastor's messages (CD's or e-copies)
6. JAMB scholarships
7. Pastor Modele's Book - This Day Forward

Prizes would be determined by an attendance card

which was shared during the 12DG on which each
member indicated their names and the bus captains
signed the attendance card for the days each member
was present.
 Please find below the categories of persons
who came for the 12DG and successfully
submitted their attendance sheets.

Present for 12 days - 25 people

Present for 11 days - 13 people
Present for 10 days - 8 people
Present for 9 days - 13 people
Present for 8 days - 6 people
Present for 7 days - 9 people
Present for 6 days - 12 people
Present for 5 days - 6 people
Present for 4 days - 7 people
Present for 3 days - 3 people
Present for 2 days - 4 person
Present for 1 day - 7 people

PRIZE CATEGORIES
For members present for:
12days of 12DG:
- Winner: COZA TV decoder, 32-inch TV, and installation
- 1st runner up; Book (This Day Forward), 1Month Subscription of
Pastor's messages, and 1 COZA T-shirt
- 2nd runner up; 1 COZA T-shirt and 1Month Subscription of
Pastor's messages
 11days of 12DG:
- Winner: Pastor's messages and 1Tshirt
- 1st runner up; Pastor's messages and 1Tshirt
- 2nd runner up; Pastor's messages and 1Tshirt

For the remaining 22 persons that came

consistently for 12 days and 10 persons that came
consistently for 11 days of the 12DG, they would
be given a COZA branded Jotter and a pen with a
copy of Pastor's message.

Jamb Scholarships:
- This scholarship would be given to 6 persons who
have attended all 12 days of the 12DG. This
scholarship would be subject to the following
requirements;
Complete attendance of all 12 days of the 12 days
SSCE or WAEC result
