JOINT AFFIDAVIT of COHABITATION

WE, RHEA E. LAGUISMA and BRYAN O. DUGA, both of legal age,

Filipinos, common-law spouses and residents of Barangay Pangobilian,
Brooke’s Point, Province of Palawan, under oath state that:

1. We have been cohabiting and living together as husband and

wife since 2015 in Brgy. Pangobilian, Brooke’s Point, Palawan.

2. Considering our mutual desire to strengthen our bond, we

intend to enter into a contract of marriage;

3. We hereby declare that we had been living together for a

period of more than five (5) years without any impediment to marry each
other;

4. We are executing this affidavit freely and voluntarily, to attest

to the truth and veracity of the foregoing statements.

IN WITNESS WHEREOF, we have hereunto set our hands this 17th

day of November 2022 at the Municipality of Brooke’s Point, Palawan.
