PLDT Inc. has commissioned Huawei Technologies Phils., Inc. (Huawei) and its sub-contractor to
do its telecommunication service improvement program, PLDT FiberGen Projects, using Fiber
Optic Cable (FOC) within your Jurisdiction. Hence, we respectfully request for a WORK PERMIT/
CLEARANCE allowing us to conduct the following activities:

1. FOC survey, verification and installation to replace defected portion.

2. Provision and Activation of Aerial Fiber Optic Cable (FOC) Installation
3. Testing and Splicing of existing and New FOC
4. Pole erection if needed

Rest assured that we will conform with the safety requirements during the entire project
implementation and deliver the state-of the-art Telecommunication Services to the community we
both serve.
