    Oral Health School Screening


    IMPORTANT: If your child is not in JK, SK,
    Grade 1 or Grade 2 but you would like them
    to be screened when Public Health is at your
    school, please send a letter or note to your
    child’s teacher.
    If your child is in JK, SK, Grade 1 or Grade 2
    and you do not want your child to be included
    in the Public Health dental screening, please
    send a letter or note to your child’s teacher.

    NOTE: If your child has an oral health problem


    (cavities, pain or infection), please call
    Southwestern Public Health to make an
    appointment. You don’t have to wait for the
    school screening visit.

    WHO? WHERE?
    • Students in JK, SK, Grade 1 and Grade 2 • Most screenings are done in school but can
    (with additional grades possible) also be booked at Southwestern Public Health.
    • Any additional student by parent request
    WHY?
    WHAT? • We link eligible children to FREE oral health
    • Oral health screenings by Registered Dental treatment programs. Our oral health staff
    Hygienists. perform screenings in accordance with the
    Ontario Ministry of Health and Long-Term
    • Oral health screenings are visual assessments Care’s Mandatory Health Programs, under the
