    ACP Application
    Bring the Following:
    1. Government ID (Drivers License,
    CA ID, Military ID, etc.)
    2. NSLP or CEP Letter AND Student
    Report Card.
    3. Proof of qualifying program such
    as SNAP (CalFresh), Medicaid,
    WIC, etc.
    4. If you are current Internet
    subscriber, please bring your
