WISe is a new approach to helping Medicaid-eligible youth with mental health challenges succeed.
Youth receive therapy, a team, and other services and supports to help them reach their goals.
Services are available at home or in a community setting with the overall goal to support you and your
family.

WISe is an intensive and new holistic method of engaging Medicaid-eligible youth with complex needs
(this includes children, youth, and their families) so that they can live in their homes and communities
and realize their hopes and dreams by providing supports like therapy and other services.
WISe is available to Medicaid-eligible children and youth 20 years of age or younger with complex
emotional, behavioral, and social issues who meet medical necessity criteria for WISe services.

How is WISe different than other services?
WISe teams recognize that youth need to have a voice and choice in their treatment. This means that
youth are given the information they need to be empowered to make positive decisions regarding their
care and treatment in the WISe process.
With the support of your WISe team, you will set your own goals and then make a plan to achieve them.
Your WISe team will build on what is going well in your life and what you are good at, not just the
challenges you are facing.
WISe crisis services are available 24 hours a day, 7 days a week. You will be able to get help during a
difficult time from someone who understands your situation and can come to your home, school, or
wherever you are most comfortable

Who would be on my team?

Care Coordinator
I facilitate the treatment and communication between the team and other community services and
supports.
Therapist
I provide behavioral health services to the youth and family so they can overcome challenges and have
the tools they need to succeed.

Who can receive WISe?
If you are a youth over the age of 13: you can ask for a WISe screen to find out if you are
eligible.
If you are a youth under the age of 13: you need permission from your parent or guardian.
If you are a parent: you can refer your child for a WISe screen at any time, but after age 13, a
child must consent to services.
