by Immigration, Refugees and Citizenship Canada (IRCC) at the Case Processing Centre in
Mississauga on behalf of the following family member(s):

Please note that the following documentation will be required for your sponsorship
application to determine whether you meet the minimum necessary income requirements to
sponsor a parent or grandparent. In order to minimize processing delays, if not already included
in your application, please obtain and submit as soon as possible:

 A completed and signed authorization form – IMM 5768 - Financial Evaluation for Parents
and Grandparents Sponsorship providing your Social Insurance Number.

o If you selected “Yes” to the Statement of Consent on this form and provided your
Social Insurance Number, you authorize IRCC to request information from Canada
Revenue Agency (CRA) in regards to your, or the cosigner’s (if applicable), income
tax information for the three consecutive years immediately preceding the received
date of your application.

OR

o If you selected “No” to the Statement of Consent on this form then you would need to
provide in addition, a completed form - IMM 5748 - Income Sources for the
Sponsorship of Parents and Grandparents and an original OPTION C-Printout
for the three consecutive years immediately preceding the received date of your
parents and grandparents sponsorship application. This document is not the same
as the Notice of Assessment and the Option C available on your My Account.
The Option C document is available free of charge by calling the CRA. To obtain this
document please call CRA at 1-800-959-8281.

If you are a resident of Quebec, you are not required to provide the above documents. You must
meet Quebec’s immigration sponsorship requirements if sponsorship eligibility is met. The
ministry in Quebec designated to immigration will assess your income.

Please note:

 Any documents submitted which are not in English or French must be accompanied by a
notarized translation in either English or French.
 Please send us scanned or photocopied versions of the requested documents. Please do
not submit originals, unless otherwise stated, as documents submitted will not be
returned.
 Please note that we can only accept unzipped PDF, JPG or .doc files that are attached to
an email, totaling less than 5 MB.

When should I contact IRCC?

You must notify IRCC of any changes to your application. Examples of changes include:
 Changes in your personal circumstances, such as birth or adoption of a child, death of a
family member, a marriage or a divorce;
 Change of contact information (e-mail, mailing address, telephone number);
 Appointment or change of immigration representative (IMM 5476) or designated individual
(IMM 5475);
 Decision to withdraw your application.

You have the option to withdraw your application at any point in the process, provided that a final
decision has not been made on your relative's application for permanent residence. You may be
eligible for a refund of the sponsorship fee or the permanent resident application fee if processing
of your file has not yet begun.

You must report any changes to your address, telephone number(s) and email address
immediately. Please note that CPC Mississauga will not update contact information. Changes to
your contact information should be made through our online service at
www.cic.gc.ca/english/information/change-address.asp.

How do I check the status of my application?

You may use your Unique Client Identifier (UCI) number located at the top left of this
correspondence to check the status of your application on the IRCC website at
https://www.canada.ca/en/immigration-refugees-citizenship/services/application/check-
status.html.

How long will it take to process my application?

Processing times vary. You can consult the processing times by visiting the IRCC website at
www.cic.gc.ca/english/information/times/index.asp. Please note that, given the high volume of
inquiries submitted to this office and Canadian visa offices abroad, status updates on cases are
not normally provided unless the processing time for a file is longer than the service standards
outlined in the IRCC website.
