NARRATIVE REPORT

The Level II Bachelor of Science in Nursing Students, group 1 of Calamba Doctors

College had their last hospital duty exposure for the second semester at Dr. Jose P. Rizal

Memorial Hospital last February 27, 2023, Monday from 2:00 pm to 10:00 pm shift under the

care of their clinical instructor Mr. Rene Mundin R.N., M.A.N.

The last day of our duty. When we arrived at the hospital, as usual we had to sign our

attendance sheet with our name, date, time and signature. We make sure to arrive early before

our scheduled time because as student nurses, we are taught to be punctual, early and always be

on time. Before we start the activity that we are going to do, we are asked to get ready and sort

our things. In our previous duty we were assigned to the third floor but this time we are going to

the second floor where the operating room and delivery room is.

At the nursing station we were overwhelmed because we saw how many patient charts

there are. Also upon seeing the room where the patients are staying we saw that two patients are

sharing a bed due to lack of adequate facilities. After touring the room with the nurses and our CI

checking the patients. we start to gather the patient's name to distribute to the group. We had 12

patients, each member will handle 1 only. From 4:00pm to 5:00pm, we began our nursing

rounds, vital sign taking and nurse patient interaction. When we entered the room the first thing

we did was introduce ourselves and which school we were from. Then start our NPI asking the

patient's name for verification purposes. Next is assessing the patient by observing from head to

toe, asking for chief complaints, DOA and the patient's medical and family history. Lastly we
take the patient’s vital signs which are the pulse rate, respiratory rate, temperature, blood

pressure and o2sat. After that we bid our goodbye telling that we are going back before 8:pm for

another vital sign taking.

Furthermore, from 5:00pm to 6:30pm we need to review the patient's chart then take

notes on important details such as medication, diet, diagnosis, chief complaint and ect. But today

we are not asked to do it b
