Eighth-grade Field Trip

Friday, May 13th, eighth-grade students will be participating in a field trip to Universal Studios
and Islands of Adventures in Orlando, FL. Please read and complete the information below then
return the signed document to your child’s homeroom teacher by Tuesday, May 3rd.
Every student is required to have this signed document on file prior to the field trip.

1. All students participating in the field trip are required to ride the school bus leaving San
Jose Catholic School at 6:15 a.m. on Friday, May 13th.
2. Attendance is taken, and students are in the care of their teachers.
3. Upon arrival to the parks, chaperones are responsible for students in their care.
Students are reminded that they are representatives of their school and conduct
reflecting such is expected.
4. Should any issues arise, the chaperone(s) must communicate immediately with the
teachers.
5. The field trip will conclude in Orlando at 6:00 p.m. Parents must sign one *option below:

*My child will be returning to San Jose on the school bus at the conclusion of the field trip.

*I am chaperoning and plan to keep my child following the conclusion of the field trip. I
understand I am required to sign my child out with the teacher after chaperone duties have
been completed. I acknowledge that if an emergency should arise and I cannot bring my child
home, he/she will return to school on the bus.

whom I am on their dismissal form in the office:
