1. INTRODUCTION

1.1 PURPOSE OF PLAN

The purpose of this document is to ensure that adequate Safety, Health and Environmental
strategies are in place during the works that as to comply with Indonesia Manpower Ministerial
Regulation. The Health, Safety & Environmental Management Plan (HSEMP) shall in conjunction
with the Shipyard’s Safety Management Systems.

The objective of this plan is to enhance the COMPANY and PT ASL Shipyard Indonesia safety
and work procedures in an effort to ensure best practice and integrity are achieved.

The Shipyard shall fully implement the elements of its Health, Safety and Environment
Management Systems including those that are listed below:

• Shipyard HSE Manual


• Shipyard Emergency Plan
• Job Safety Analysis (JSA) & Risk Assessment (RA)
• HSE Induction Programme
• HSE meeting and walkabouts

1.2 HSE MANAGEMENT SYSTEM

The PT ASL Shipyard Indonesia commitment to HSE is embodied in the PT ASL Shipyard
Indonesia HSE Manual and is communicated to all employees and posted on all Departmental and
Site Notice Boards. It is the policy of PT ASL Shipyard Indonesia that HSE are of prime importance
to all personnel who work for or on behalf of PT ASL Shipyard Indonesia.

The PT ASL Shipyard Indonesia HSE Management System is structured so that the activities of
the principal departments within the Company enable the HSE Objectives and Targets to be met.
The hierarchy of the PT ASL Shipyard Indonesia’s HSE Management System has two levels of
documentation;-
• PT ASL Shipyard Indonesia HSE Manual– Defines responsibilities and processes involved in
the management of HSE.
• Working Procedures relating to HSE (HSEMP) – Provide overall HSE procedural and
operating requirements.

1.3 LEADERSHIP AND COMMITMMENT

All levels of management will have a direct involvement, in the implementation of the HSE
Policy and demonstrate commitment to the Health, Safety and Environmental Management
Program as a function of their daily responsibilities.

Corporate and Project Management shall:

• Ensure that all their decisions and practices are in line with the principles of the HSE
policy.
• Provide adequate resources for health, safety and environmental matters comply with
COMPANY HSE minimum requirement standard which applicable to the work
• Participate and attend HSE meeting, also COMPANY HSE meeting at site if required

• Participate in the safety audit program and hazard identification systems.


• Visit work areas minimum 1 time during contract life and be seen to wear the required
personal protection equipment.
• Deal appropriately and immediately with any non-compliance with safety rules or
procedures they observe.
• Make safety as the first item of agenda in progress meeting agendas.
• Participate in accident investigation and review of accident reports and remedial actions.
• Provide an appropriate climate for participation in HSE management for all levels of their
work groups.
• Make sure all PT ASL Shipyard Indonesia and Contractors employees at site comply with
the HSEMP and its objectives.
• Monitor the HSE performance of all groups in their area of control.
• Actively promote HSE management at all levels.

1.4 PROJECT HSE POLICY AND OBJECTIVES

The PT ASL Shipyard Indonesia project objectives are to provide safe and healthy working
conditions for all employees and to maintain the highest HSE standards in relation to safe systems
of work and equipment operations. Through training, supervision and the provision of
information, PT ASL Shipyard Indonesia is committed to ensuring a safe place of work for both PT
ASL Shipyard Indonesia employees and others who come into contact with its activities.

The company’s ultimate objectives to have an “injury and illness free workplace”, this has
been acknowledged by PT ASL Shipyard Indonesia and will be incorporated into the way PT ASL
Shipyard Indonesia will conduct its HSE Management activities.

To achieve these objectives the combined efforts of the shipyard and COMPANY personnel
shall be required. All parties shall be working together to identify and control and manage the
risks associated with the works. The custodian of this plan on behalf of the Project Manager is the
Project Site HSE Officer/Assessor.

2. SMOKING POLICY

All personnel including COMPANY Owners and contractors, PROJECT/VESSEL NAME ship’s crew /
Yard personnel and contractors are strictly prohibited from smoking on board vessel. Any person
found smoking on board should be reported to the Project Manager/HSE Assessor for disciplinary
action. Smoking offence shall result in immediate dismissal of the offender from the project.
COMPANY superintendents/representatives shall only smoke in the designated area inside the
yard.

3. ALCOHOL AND DRUG POLICY

No alcohol or drugs must be taken into the worksite. All personnel COMPANY Owners and
contractors, PROJECT/VESSEL NAME ship’s crew / Yard personnel and contractors, must never be
unfit to work due to alcohol or drug usage. Any person found in possession of any quantity of
alcohol or drug shall be disciplinary dealt with, which could result in instant dismissal. Anyone
who is found suspect of taking any drugs or alcohol, the suspect may be subject to being
searched and instructed to perform blood/urine tests.

5. PROJECT HSE PLAN

The Project HSE Plan details the HSE management system that will be in place for the project,
other site facilities and related subcontractors to assure that the HSE aspects of the projects
are adequately managed and that the activities are conducted in line with COMPANY and PT
ASL Shipyard Indonesia requirements. It is further intended that Company’s safety initiatives
and HSE POLICY will also be incorporated into the Project HSE Management Plan.
This Project HSE Plan identifies the key HSE related activities and deliverables, in accordance
with the requirements of legislation, regulations, COMPANY. The Project HSE Plan will serve
as a “living document” and PT ASL Shipyard Indonesia will update it as required to reflect
changes and progress towards completion

6. ASL PROJECT MANAGEMENT ORGANIZATION

PT ASL Shipyard Indonesia is an organization structured to emulate the progression of


facilities from engineering process until Projects hand over. The Project Management Team
for the Project.

7. ROLES AND RESPONSIBILITY

The HSE management arrangements outlined in this document are implemented through line
management. Appropriate project HSE resources are provided to assist in fulfilling these
responsibilities. PT ASL Shipyard Indonesia shall also ensure that all personnel are informed
and aware of their roles and responsibilities.
The Project Manager has overall responsibility for the HSE performance of the project with
the QHSE Manager or Asst. HSE Manager providing advice and support and coordinating the
HSE activities. An overview of the responsibilities for the PROJECT/VESSEL NAME project
programmed is outlined below.

7.1 SENIOR MANAGEMENT

Senior Management comprises of management personnel from QHSE Manager or Asst. HSE
Manager to above who can make policy decisions on safety for the yard as a whole. The
overall responsibility towards safety lies with the senior management. In order to ensure that
high standards for safety are set and maintained, they should:

• Formulate and implement an effective safety and health policy for the whole yard.
• Review the policy continually and update periodically so as to accommodate changes.
• Allocate adequate resources for implementation of the safety policy.
• Develop training and competency for ASL and Sub-contractors.
• Ensure all HSE and safety staff is trained to a level to enable them to fulfill their task
requirements.
• Set development plans for HSE and Managers.
• Set achievable targets, conduct periodic reviews.

7.2 HEAD OF DEPARTMENT (HOD)

HOD will ensure that the day to day compliance of the OH&SMS along with the HSEMS. All
safety notices and tool box meeting will be collated and ensured that all projects have
complied. HOD will be aware of all permits being issued for each project within the ship yard
to ensure no conflict between projects. HOD will promote a high degree of safety awareness
among all personnel, including subcontractors and ASL personal. This involves a number of
key responsibilities as follows:

• Administer the company safety policy on a daily basis.


• Convene safety meeting as stated in this manual. Meetings are held for all personnel
with records of attendance and minutes of meeting kept.
• Ensure that the work force under their control is adequately informed on risk control,
trained and competent for the operations which they are required to perform.
• Arrange for the scheduled training of staff in aspects of safety relevant to their works.
• Ensure that all statutory and company examinations and inspections of yard
machinery, plant etc. are carried out and relevant registers are completed,
maintained.
• Ensure that all statutory notices, regulations, sign etc. are displayed and maintained at
relevant locations.

• Ensure that emergency procedures are implemented; periodic exercise is held to


familiarize all concerned on the actions required.
• Ensure that the arrangements for first aid and other emergency services are provided
and are known by employees.
• Carry out work place safety inspection tours.
• Ensure full co-operation of the protection personnel with the safety personnel and
take action upon any safety matter, which has been brought to their notice, which
demands their attention.
• Ensure that all incidents involving injury, damage to property as well as near miss are
reported to HSE Department and are thoroughly investigated to stabilize the causes so
as to prevent accordingly.
• Before hazardous operations commence, procedure job safety analysis or method
statements to detail the preventive measures required to control the specific risk.
• Ensure that subordinate receive clear safety instructions regarding the hazardous and
the associated preventive measures under which the operations will be carried out.
• Ensure that sufficient safety protective equipment is available and where necessary,
employees are given personnel issues of the necessary personnel protective
equipment.
• Promote Safety awareness through leading by example.

7.3 PROJECT MANAGER

• Maintains safe and healthy working conditions and practices and also fire prevention on
board ship under his charge.
• Ensure that PT ASL Shipyard Indonesia HSE Policy and the HSE Management Plan for the
contract are implemented including stopping work for non-compliance.
• Enforces all legislated HSE requirements and PT ASL Shipyard Indonesia HSE rules and
regulations.
• Liaise and co-operates with COMPANY Superintendent and PROJECT/VESSEL NAME
ship’s crew in the maintenance of a safe and healthy work environment on board the
vessel and elsewhere in the yard including in sub-contractor work areas.
• PJM shall conduct the daily Vessel Safety Co-ordination Committee (VSCC) meeting and
Production meeting.

7.4 QHSE MANAGER / ASST. HSE MANAGER

• Enforce all HSE procedures and instructions including stopping work for non-
compliance.
• Advise management on all matters pertaining to HSE including compliance with local
HSE legislation.
• Plan and implement HSE programmes and promotional activities that will improve HSE
awareness.
• Initiate investigations into all accidents, recommend corrective and preventive actions
and report all reportable accidents to the relevant authorities.
• Brief and instruct sub-contractors management on the HSE procedures and
instructions that they and their employees are required to comply with.
• Deploy HSE Department personnel to monitor and maintain HSE standards, prevent
and fight fires, respond to accidents and control environment pollution.

7.5 PROJECT HSE LEAD

Project HSE Lead are responsible for conducting all site HSE activities for PT ASL Shipyard
Indonesia and for monitoring all contractor HSE activities at the respective worksite, to
conduct Investigation of all accidents and incidents in the project.

7.6 LINE MANAGEMENT


Line management consists of a team of supervisory and technical staff such as engineers,
supervisors and foreman who is headed by departmental manager or project manager who
have the operational responsibility for implementing the required standards of safety within
their sphere of control. They are expected to promote a high degree of safety awareness to
all personnel by, leading by example in respect to wearing PPE and safe operating standards.
This involves a number of key responsibilities as follows:

7.7 INDIVIDUAL EMPLOYEES

• Perform their work activities in the manner in which they have been instructed and
trained such that they do not place themselves or anyone else at risk.

• No attempt to do any work, operate any equipment or machinery that are dangerous
in nature, unless they have been trained and authorized to do so, in accordance with
recognized procedures.
• Use equipment according to its function; ensure that it is maintained in a safe
condition. Report any known defects to their Supervisor.
• Use personnel protective clothing and equipment provided according to their
functions in order to minimize injuries.
• Report any unsafe conditions, incidents and near miss to their immediate supervisor
so that remedial action can take to eliminate risk.
• Report any shortcomings in the protection arrangements to their immediate
supervisor as soon as these become apparent.
• Use the main accesses as provided, comply with the relevant safety notices and signs
and do not enter any area where they may be placed at risk or do not have
authorization to enter.
• Keep their work place in a safe and tidy condition.
• Develop a personal concern for safety and if faced with any doubts to their ability to
perform a task safely, they must inform their supervisor

7.8 HSE ASSESSORS / HSE OFFICER


• Conduct regular inspection on their respective areas and make sure the standards and
procedure are implemented.
• Ensure that all required permit to works are thoroughly inspected and approved by
Project Managers.
• No attempt to do any work or allow any works so to operate any equipment or
machinery that are dangerous in nature, unless they have been trained and authorized
to do so, in accordance with the OH&S Management System.
• Enforce safety and health procedures and practices of the yard.
• Ensure that all workforce are equipped by personal protective equipment according to
their jobs.
• Conduct investigation if any unsafe conditions, incidents and near miss on their
respective vessels/projects.
• Actively participant in the ASL observation program to record and minimize the risk of
occurrences.
• Ensure that their work areas are safe and tidy condition.
• Promote safety and health procedures and practices in the yard.
• Conduct safety briefing and toolbox talk on-site.
• Secretary of Vessel Safety Co-ordination Committee (VSCC) meeting.
• Participating on any emergency or drills arise on site (Emergency Response Team).
• Implementing stop work if any danger or hazard to the life, property and environment.
• Conduct Risk Assessment or JHA.
• Impose penalty that will violate safety and health procedures and practices or would
endanger life or property.

7.9 HSE PROMOTERS


• Understand the task they are required to do.

• Be active during the toolbox talk and safety briefing.


• Conduct daily HSE inspection on their vessel, workshop and fabrication.
• Report and convey any and all potential safety concern.
• Monitor all the activities on board, workshop and fabrication.
• Report any unsafe act, unsafe condition, near miss and incident to their HSE Assessor
or QHSE Manager and Asst. HSE Manager.
• Ensure that all workforce are equipped by PPE.
• Ensure that all safety equipment is maintained in a working order and report any
failures or concerns of equipment.
• Keep a known and obvious presents on the work site to act as clear reminder to other
contractors.
• Ensure that Permit to work system are implemented.
• Stop work if any endanger in life, property and environment.
• Conduct gas free inspection in all confined spaces.
• Respond immediately if any emergency occurred in the yard premises or vessel.
• Participating on any emergency or drills arise on site (Emergency Response Team)

7.10 FIRE AND RESCUE TEAM

• Maintain all fire and emergency equipment be it the HSE store or out on site.
• Check and test environmental hazard containment equipment.
• Maintain records of regular equipment checks in office area.
• Conduct regular training exercises and record any action plans so to improve efficiency
and speed.
• Train HSE staff in and during training exercises.
• Record safety walk around and action taken.
• Conduct safety checks of all building and record with action taken
• Actively participate in ASL Observation program and record weekly and monthly
results
• Maintain a record of fire extinguisher with location and condition.
• Ensure regular inspection conducted on fire engine and pumps weekly and monthly
and recorded, to include all equipment carried onboard.
• Record and investigate all accidents and incidents to establish the causal factor to
prevent re-occurrences.
• Conduct training to the individual’s professional ability.
• Establish a schedule of special training of selected equipment and scenarios.
• Ensure all first aid and medical boxes are maintained and notify Medical team of same.

7.11 MEDICAL AND FIRST AID TEAM


• Regularly promote and conduct first aid refresher classes for trained first aiders.
• Promote and practice personal hygiene
• Deliver treatment as required
• Maintain a record of all medical supplies date of expire etc.
• Record all medic at treatments for monthly statistical records

7.12 CONTRACTORS

• Compliance with the general requirements of the company OH&S Management


System and any legislative requirements particular to their work activities.
• Communicating the OH&S requirements of both their own and company OH&S
Management Systems.
• Ensuring that any conflicts and/or deficiencies between their OH&S standards and
those given in OH&S Management System are resolves prior to the commencement of
any work or activities.
• Providing and agreeing an OH&S plan with the HSE coordinator appropriate to the
nature of the work to be completed.
• Ensuring that the OH&S risk associated with their work activities have been assessed
and appropriate control measures are in place to prevent or adequately control.
• Developing current and up to date OH&S Policy, risk assessments, method statements,
safe systems of work and any other.
• OH&S systems, guidelines and/or procedures available for review prior to and during
the completion of work.
• Ensuring any significant changes to their OH&S Policy, Management System, etc. is
communicated to shipyard management.
• Participating in reviews of OH&S performance during and on completion of the
contracted work or activities.
• Ensuring legal compliance at all times.
• Reporting OH&S incidents/accident, near miss involving themselves, their employees,
the public or other affected by their work activities. These must be reported to the
Shipyard HSE Department.
• Participating to all HSE programs, campaign, safety briefing, trainings and etc. that the
shipyard implemented.
• Ensuring that their employees and workers are equipped with the appropriate
personal protective equipment to their specific task.
• Promote the safe conduct of work in the Shipyard and on board the Ship.
• Liaise with the PT ASL Shipyard Indonesia HSE personnel on matters relating to the HSE
and Welfare of persons working in the shipyard and on board the Ship.

7.13 CUSTOMERS
• Ensuring that all contractors/vendors employed directly by them to within the
shipyard to attend the HSE Induction course.
• Ensuring that their crew and anyone contracted by them are aware of and comply with
all the requirements of the company OH&S Management System and Procedure.
• Informing of any hazardous conditions onboard vessel prior to the vessel entering the
shipyard premises and work commencing.

8. HSE ORGANISATION

PT ASL Shipyard Indonesia recognises its responsibilities to provide a safe and healthy work

9.2 VESSEL SAFETY CO-ORDINATION COMMITTEE MEETING (VSCC)

A daily VSCC meeting shall be conducted on board and will be chaired by the Shipyard’s PJM.
The VSCC shall include the following members:

• Shipyard HSE Assessor/ Officer who shall be the secretary


• Shipyard Trade Engineers/Foremen (1 per trade)
• COMPANY / Owner or his nominated deputy

9.3 TOOLBOX TALK MEETING

Supervisor will conduct Safety Tool Box meeting with the employees, for which they are
responsible, prior to the commencement of each day’s work activities. This meeting should
be brief, informal and a means for the Supervisor to allocate work tasks for the day whilst
providing a forum for reinforcing HSE matters and the appropriate equipment and materials
needed to undertake the safely and productively. No records are maintained for this activity.
If particular incident has occurred where there has been an injury or a near miss event, the
Supervisor will call a toolbox meeting of his personnel to briefly discuss the event,
consequences, and remedial action taken for such specific cases, records shall be maintained
in a brief note form and available to all participants, with any action items arising noted and
the action taken referred back to the work group as soon as possible. See APPENDIX B

9.4 SAFETY BRIEFING

The function of the safety briefing is to create safety awareness and to impact relevant safety
instruction to the workforce, including contract workers.

all levels within the organization, including its subcontractors, to maintain an injury free and
healthy work environment and to improve HSE performance.

9.6 SAFETY INCENTIVES

A safety incentive program will be established at each site to recognize positive behavior and
attitude by personnel to HSE issues. The objectives of the incentive scheme are to promote
and encourage safe behavior by individuals and teams.

In particular we are not measuring lagging indicators; in this case we are measuring inputs and
proactive behavior and action.

The incentive scheme will recognize:

Leadership at management, superintendent and supervisor level in providing positive


examples of HSE in managing the yard and work teams. For management this may include
visibility at the worksite in HSE inspections, discussing HSE with workmen, attendance at
safety meetings etc.

For supervision involvement in leading risk assessment studies, following work permit
requirements, leading safety meetings, JHA’s and promoting HSE to the workforce

11.1 PERSONNEL PROTECTIVE EQUIPMENT

Appropriate Personal Protective Equipment (PPE) shall be correctly worn/used by all


personnel that are involved in this project at all time. All personnel must wear the minimum PPE
whilst working at the site of:

• Safety helmet;
• Safety spectacle/glass;
• Safety boots/ shoes;
• Safety belt / body harness;
• Respirator;
• Hand gloves;
• Overall (for PT ASL Shipyard Indonesia employees), sub-contractors to use appropriate
work clothes comprising either boiler suit or long trousers and work jacket. Tee shirts
will not be acceptable.

In addition, hearing protection, gloves or any other necessary PPE will be worn as required
according to the work being done or the prevailing conditions. Specific PPE requirements i.e.
appropriate face shields and respiratory protection shall be determined according to the nature
of work such as arc welding, grinding or work involving the use of hazardous substances in
accordance with the MSDS requirement and PPE matrix. See APPENDIX W

All personnel shall continuously enforce the use of PPE and shall take appropriate corrective
action to rectify any occurrence of non-compliance.

11.2 ACCESS, GANGWAY AND CONFINED SPACE TAG SYSTEM

• PT ASL Shipyard Indonesia’s Security Officer shall ensure that only authorised personnel
gain access to the vessel during the project.
• All personnel including COMPANY employees/contractors must have to undergone the
Shipyard’s HSE induction training by Yard HSE representative before they are issued
with two Ship Access Passes. SEE APPENDIX C
• COMPANY Superintendent/Owners, Representatives will receive a HSE Induction sticker
with serial number after the safety induction that was valid for 1 year from the date of
induction.

• COMPANY Sub-contractors must send the list of manpower to be inducted via emails to
Project Manager, Commercial, HR, HSE, Security and Customer Service Department.
• COMPANY Sub-contractors must also filled up the HSE Induction Application Form and
to bring and signed by a Project Manager, HR, Security and HSE Department. HR
Department will verify their BPJS insurance for locals and Insurance for expatriates. See
APPENDIX C
• PROJECT/VESSEL NAME ship’s crew will be given safety briefing on board the vessel
once the vessel arrive in PT ASL Shipyard Indonesia.
• One of these passes (No. 1) shall be placed on the access board, which will be
positioned at the bottom of the gangway prior to boarding the vessel.
• When entering any confined space, all personnel shall deposit their other Ship Access
Pass (No.2) into the Tally Box that is placed at the entrance of the confined space.
• Upon exiting the confined space all personnel shall remove their passes from the Tally
Box.
• Where a confined space has more than one access, the Tally Box shall be placed at the
each entrance, but entrances should be minimised to maximum of two. The tank
entry/work permit shall be displayed at the main entrance only.
• In the event of an emergency alarm, all personnel must evacuate the vessel and
assemble and await instructions from the Project Manager and Command Center.

11.3 LIFTING PROCEDURE

To ensure that all lifting operations are performed in accordance with the relevant
guidelines and safe working practices and to make sure that proper equipment.

a. Routine Lift

• A pre-lift toolbox meeting should be conducted involving all relevant parties
• All lifting points (eye lug/pad eye) must be cut by CNC machine and tested by
QA/QC NDT or UT.

PROJECT/VESSEL NAME Captain / Master must not use their on cranes without prior
approval by PT ASL Shipyard Indonesia Management and Project Manager. All PROJECT/VESSEL
NAME eye lug / pad eye must be MPI by QA/QC before it was use.

11.4 LIFTING INSPECTION

To ensure that all lifting equipment, appliances and gears are maintained, inspected and
documented in accordance with the relevant guidelines and safe working practices. All lifting
appliances and gears will be colour coded as per PT ASL Shipyard Indonesia.

11.5 WORKING AT HEIGHTS

To give instruction, guidelines and considerations to be adopted by all personnel who are
required to work at height.

This procedure applies to all employee and contractors of PT ASL Shipyard Indonesia and
COMPANY at all location on board PROJECT/VESSEL NAME.

Prior to working at height a risk assessment is to be carried out to establish what health and
safety measures needed to be adopted to avoid or reduce risk. Consideration should be given to:

• The physical condition of the people involved;


• The activity;
• The equipment to be used;
• The location – is it near water, close to power cables, etc.;
• The condition and stability of the work areas and surfaces;
• The environment, i.e. temperature, lighting, wind, etc.;
• The duration of work

11.5.1 HIERARCHY OF RISK

There exists what is termed as a “hierarchy of risk” based upon risk and practicality. The
higher up the hierarchy of access is considered to be, the lesser the risk associated with it.

• Permanent Installed Access is at the top hierarchy as it has the least amount of risk
associated with it. This involves the installation of walkways and gantries, and where
appropriate, a site that must be accessed regularly for routine maintenance.
• Temporary Working Platform such as staging, cradles and mobile platforms. This
method requires those using them to be trained before using them.
• Bottom of the hierarchy and therefore more risky is the use of Personal Protective
Equipment (PPE) and Fall Arrest Techniques. This involves the use of PPE to stop a falling
worker from hitting the ground and prevent injuries.

11.5.2 PERSONAL PROTECTIVE EQUIPMENT

It is important to recognize that the correct application of PPE is just as bad as not using
anything at all.

• Fall Arrestors

11.5.3 RULES TO PREVENT FALLING

Make sure the working platforms (scaffold) is secure. Check the following:

• Will support the weight of workers using it and any materials and equipment they are
likely to use or store on it; the safe for use (OK) tag is posted on the access ladder of the
scaffold.
• Is stable and will not overturn. Scaffold needs to be tied to a supporting structure.
Mobile Elevating Work Platforms (MEWPs) may not be safe on uneven or sloping

ground;
• Provides adequate working space; and
• Is footed on stable ground or on a stable support or structure
• Provide guardrails, barriers, etc. at open edges, including open decks/floors, roofs, and
edges of working platforms.

11.6 SCAFFOLDING

To provide a safe procedures in the erection, use and dismantling of all staging and
scaffolding. To clearly stipulate the levels of personnel trained and competent in erecting,
altering or dismantling of scaffolding.

To raise the awareness of all personnel working on board PROJECT/VESSEL NAME to the
dangers involved and the precautions to be taken when accessing or working on scaffolding.

This procedure applies to all employees and contractors of PT ASL Shipyard Indonesia and
COMPANY for all work location on board PROJECT/VESSEL NAME. It identifies the measures to
be taken to ensure that all scaffolding operations that take place are performed in accordance
with the relevant guidelines and standard safe working practices in order to reduce the risk of
incident or injury.

The Yard Manager is directly responsible for ensuring the following:

• HSE Department and contractors have carried out PTW, JSA and mitigating factors are
implemented.
• Suitable qualified “competent person” scaffolders and inspectors.
• The procedure is reviewed from time to time to ensure its validity.

11.6.1 ERECTING / ALTERED OR DISMANTLING OF SCAFFOLDING

• All scaffolding must be of good construction, of suitable and sound materials and
adequate strength and quantity for the purpose for which it is intended.
• Scaffolding must only be erected, altered or dismantled by trained and competent
personnel. It should be constructed according to recognized standards and as
recommended by the manufacturer. Scaffolding should only erected upon sound
surfaces.
• All persons involved in scaffolding erection, alteration, or dismantling must be familiar
with and wear approved fall arrestor equipment securely fastened to a strong point, in
addition all other required PPE.
• After construction or alteration of scaffolding, it must be inspected and correctly
tagged with “OK Tag” and signed by the competent inspector before other trades to
use it. See APPENDIX T
• Once erected and tagged, no other personnel other than trained and competent
person from the Scaffolding Department is allowed to modify the scaffolding. OK tag is
identified by GREEN tag, and it has been inspected and approved by qualified
inspector for safe to use.
• Whilst scaffolding structure is under construction, or during modification, the
scaffolding tag will show “DO NOT USE SCAFFOLD”. This tag is identified by RED tag.
• Whilst the scaffolding structure remains erected it is subjected to periodic inspections.
These inspections may be result of suspected damage (e.g. from strong winds), at the

request of the user or a member of HSE Department or at least every (7) days, the
scaffolding structure erected for longer periods.

11.7 DOCKING AND UNDOCKING

To ensure that each vessel docking repair and undocking comply with the safe guidelines.

• COMPANY Management will submit the PROJECT/VESSEL NAME Docking Plan to PT ASL
Shipyard Indonesia.
• Project Manager will send a copy of docking plan to Dock Master including the docking
repair specification of PROJECT/VESSEL NAME prior for arranging of keel blocks.
• Project Manager will discuss with the PROJECT/VESSEL NAME Captain / Master the
following:

• Forward draft
• Aft draft
• Longitudinal Center of Buoyancy
• Moment of Change Trim by 1cm
• Center of Gravity
• Longitudinal Center of Floatation
• Transverse metacentre

• PROJECT/VESSEL NAME Captain / Master must submit to Project Manager the following:

• Elevation view
• Plan view
• Cross section at propeller
• Keel profile
• Bilge Keel clearance

• PROJECT/VESSEL NAME Captain / Master shall comply with dockyard ballasting


condition.
• Project Manager, PROJECT/VESSEL NAME Captain / Master and Dock Master to inspect
the keel blocks are in good order and positioned according to the vessel docking plan.
• Dock Master to check the vessel draft and height of tide is suitable and safe for docking
operations.
• Project Manager will designated trade supervisors including with PROJECT/VESSEL
NAME ship’s crew representative to inspect bottom plugs, sea chest grating, anodes,
propeller rope guard, etc. prior for flooding of dock.
• Project Manager and PROJECT/VESSEL NAME Captain / Manager are responsible to sign
the checklist prior for flooding of dock. See APPENDIX N

11.8 ADVANCE SUPPLIER RELATIONSHIP (SUB-CONTRACTOR MANAGEMENT)

To ensure the competency requirements relating to all subcontractors working within the PT
ASL Shipyard Indonesia are identified, managed and suitably controlled.

• All new subcontractors to undertake activities on behalf of PT ASL Shipyard Indonesia,


will be responsible by the Subcontractor Company to fill up the Third Party Contractor

This shall applies to all PT ASL Shipyard Indonesia and COMPANY employees and
contractors responsible for planning, supervising and conducting welding and cutting operation
on board PROJECT/VESSEL NAME.

11.11 HOT-WORK TOOLS CONTROL

The control of hot-work tools to ensure that they are in good condition and comply with the
relevant statutory requirements. As fire and explosions are one of the major hazards in the
shipyard, maintenance of hot-work tools in good condition assumes great importance as they
can help prevent such incidents.

PT ASL SHIPYARD INDONESIA & OWNER’S SUB-CONTRACTORS shall:

• All hot-work tools brought into the yard shall be inspected and kept in the respective
sub-contractors store. They shall be numbered for identification. See APPENDIX U

11.12 MAINTENANCE REGIMENT

PT ASL Shipyard Indonesia Facility Department frequently involve the use of a wide variety
of plants, machinery, equipment and tools. In order that the day-to-day operations are carried
out safety and without any instruction, it is essential that the plants, machinery, equipment and
tools used be maintained in good working condition. Failure to carry out adequate preventive
maintenance could result in decrease in the efficiency and/or break down of plants, machinery,
equipment and tools and may lead to serious accident / incidents. Hence, proper HSE Equipment
Inspection shall be established to ensure that plants machinery, equipment and tools used in the
shipyard are properly maintained. The preventive maintenance program shall incorporate the
relevant statutory inspection, where necessary.

The purpose of maintenance regime is to ensure that plant machineries used within the
yard premises are safe to use.

a. HSE EQUIPMENT INSPECTION

HSE Equipment Inspection is predetermined work performed without any prior


knowledge of a defect. HSE Equipment Inspection keeps machinery/equipment in an
effective condition there by preventing an undressed breakdown.

The HSE Equipment Inspection carried out in the yard shall include the following
equipment/machinery;

a. Cranes
b. Fork Lifts
c. Service line
d. Compressor and Generator
e. Production machinery
f. High pressure pump for hydro jetting
g. Airless spray pumps
h. Pressure Vessels
i. Vehicles
j. Building and workshop

The type, frequency and responsibility for carrying out preventive maintenance is clearly
defined for each of the above machinery or equipment.

b. PRE-USE INSPECTION CHECKLIST

Machinery/equipment, which have system such as emergency control, lights, bracers,


etc. shall be checked at the start of the work prior use by the operator using pre-use
inspection checklist.

c. MAINTENANCE OF CRITICAL PARTS

Critical parts are defines as the components of machinery/equipment more likely than
others components to result in a major problem or loss when worn, damaged, misused or
improperly applied or improperly stored.

The procedure adopted in the yard with respect to maintenance of critical parts is as

revocation or on completion of the permits.

REVOCATION OF PERMIT-TO-WORK

• If, after issuing a permit-to-work in respect of any high risk work, the Project
Manager, as the case may be, is of the view that the carrying out of the high risk
work poses or is likely to pose a risk to the safety and health of persons at work in
the shipyard or on board a ship, he may order the high risk work to cease
immediately and revoke the permit-to-work.

• During the vessel movement or after the movement, all permit-to-work in respect
of any high risk work in confined spaces, enclosed spaces, engine room and pump
room are cease immediately and revoke the permit-to-work by the Manager and
HSE Assessor. A fresh application shall be made in accordance with regulation
“Application for Permit-to-Work”.

• If there is any erasure or falsification in the permit without a counter – sign by HSE
Assessor or PJM.

RETRIEVAL OF EXPIRED AND COMPLETION OF PERMIT-TO-WORK

• It shall be the duty of trade supervisor or foreman to remove the display copy
permit-to-work from the location of work. Sign of the permit-to-work indicating the
date and time. Return the permit-to-work to the HSE Assessor.

12. EMERGENCY RESPONSE PLAN

12.1 EMERGENCY RESPONSE

During the period that the vessel is docked, the following shall apply in relation to emergency
response:

a) Emergency response and control capabilities shall be provided through the Shipyard
Emergency Plan.

b) Emergency response can be initiated by:

• Activating portable alarm system that shall be set up on main deck


• Activating the crane siren by the crane operator

c) PT ASL Shipyard Indonesia shall provide a safe muster area that will be clearly indicated
for the emergency evacuation of all personnel. The muster area shall be sufficient to
accommodate all personnel working on the vessel.

d) Emergency alarm shall be identified as a continuous activation of the crane siren until the
arrival of the yard’s fire engine and ambulance.

12.2 EVACUATION PROCEDURE

Upon hearing the emergency alarm:

• All personnel shall immediately leave the vessel by the shortest route.
• Security and HSE personnel shall direct immediately all personnel to the safe mustering
area.
• HSE Assessors shall take initial control of the emergency situation prior to the arrival of
the QHSE Manager or Asst. HSE Manager.

The Shipyard’s HSE personnel on board the vessel shall perform the following activities:

• On observing an emergency, evaluate and call Shipyard HSE Dept. for assistance.
• On hearing the emergency alarm the Shipyard’s HSE personnel shall activate Emergency
Response Team (ERT), at the same time shall initiate the evacuation of all personnel.
• Checked confined space tag boxes and sound air horn at space entrance

All personnel vacating confined spaces must remove their entry tags/passes from the tally
box placed at the entrance of the confined space and then proceed to their mustering points.
Respective Foremen/Supervisors shall check that all their workers are accounted for and report it
to the HSE Assessors. Any missing personnel shall be reported to the HSE Assessors immediately
with the last known position on the vessel. COMPANY Superintendent and PROJECT/VESSEL
NAME Captain/Master shall also report the head counts of their own personnel and contractors
to the Yard HSE Assessor and/or the command post.

A drill shall be conducted within the period of the vessel docking, to assess the yard’s
preparedness to response to an engine room fire, emergency mustering and the ability to
account for personnel.

12.3 FIRE FIGHTING

The vessel shall be protected by fire points with charged lines of firefighting hoses rigged up
on board. A fire watchman with portable fire extinguishers shall be provided at hot work
locations where necessary.

Fire patrols are provided on a 24 hours basis. Easy communication through telephone or
radio/ walkie-talkies shall be maintained with the fire station, which is continuously manned.

12.4 MUSTER POINT YARD LAYOUT

12.5 MEDICAL EMERGENCIES

a) All personnel shall obtain first aid/medical treatment from the PT ASL Shipyard
Indonesia Medical Centre that is manned by two qualified industrial nurses on a 24
hours basis.
b) PT ASL Shipyard Indonesia shall maintain an equipped ambulance on 24 hours basis.
c) Eyewash station shall also be made available in the Yard’s Medical Centre.

13. MEDICAL HEALTH EXAMINATION / INDUSTRIAL HYGIENE


Occupational health will be considered through all phases of the Project. PT ASL Shipyard
Indonesia will ensure that adequate provisions are made to ensure that all personnel involved
in the scope of work are protected from exposure to hazards which may affect their
immediate and future health. Implementation will be administered by the following means:

• Periodic Medical Examinations to ensure personnel are medically fit for their job to be
undertaken in accordance with the relevant applicable legislation/guidelines.
• The implementation of identification of hazardous operation will limit exposure to
hazard.
• Personal protective equipment will be made available when other methods of
protection can be implemented.
• Developing operational control procedures for the Fabrication activities.
• Maintaining a safe and healthy place of work.
• Adequate clean and well maintained sanitary facilities within reasonable access to work
and rest areas.
• Adequate facilities for hand washing adjacent to dining rooms and sanitary facilities.
• Workers will be provided with covered dining facilities wherever practicable and these
will be kept clean on a daily basis.
• PT ASL Shipyard Indonesia, through its medical facilities, posters, notices and meetings will
dispense information and instructions in matters of personal and occupational hygiene
and the prevention/avoidance of disease, work related health problems and heat related
stress.

Shipyard’s personnel. The working hours shall be as follows:

14.3 HAZARD COMMUNICATION

The following Pre-Order Checklist will assist to minimize the risks to employees and the
Company:

• Obtain Material Safety Data Sheet (MSDS) from supplier or manufacturer. If information
is inadequate to make a decision, request additional information.
• If in doubt, seek advice through the HSE Officer/ QHSE Manager or Asst. HSE Manager.
• Is there a safer alternative?
• Check on statutory requirements for transportation, storage and use. Can they be met?
• Consult with employees who might be affected. Plan safeguards and safe work system.
• Provide the necessary training.
• Place order.
• On Receipt, check the material received is as ordered.

14.4 HSE NOTICE BOARD

a) A free standing HSE Notice Board showing HSE Weekly and Up-to-date statistic of the
project for PROJECT/VESSEL NAME.
b) HSE notice board for safety posters, signboard and etc.

14.5 PREVENTIVE SLIP, TRIP AND FALL

To provide guidance to all employees of PT ASL Shipyard Indonesia and COMPANY on


preventing injuries from Slips, Trips and Falls.

All work area, both on board PROJECT/VESSEL NAME and ashore, should be considered as
potential slips and trips hazardous areas and appropriate safety precautions should be observed.

14.6 SAFETY SIGN

A number of operations in PT ASL Shipyard Indonesia involves employees of different


nationalities. It is therefore necessary that certain basic safety information is communicated to
them in a manner that could be easily understood.

Safety signs or symbols use in the shipyard and on board PROJECT/VESSEL NAME are in two
languages i.e. English and Bahasa Indonesia.

15. EVALUATION AND RISK MANAGEMENT

15.1 RISK ASSESSMENT

Risk Assessment will be undertaken during before site activities. The objective of the RA will
be to identify inherent hazards associated with the Lifting activities and develop the means to
eliminate the hazard and suitable Personal Protection Equipment being used for any work
activities, or to reduce the risks posed by the hazard to as low and reasonably practicable level.

The RA will identify the activities, equipment and responsibilities for the management of
hazard and risk within each work activity. The results of the RA will detail in the HAZID register
prepared by PT ASL Shipyard Indonesia. SEE APPENDIX D

The RA process typically includes the following activities:

• Break the activities down into individual jobs and steps.


• Identify hazard.
• Develop means of eliminating the hazard or, of reducing risks to tolerable and
manageable levels.
• Record RA results.
• Incorporate recommendations into fabrication sequence and load out procedure.

16.3 HSE STATISTIC

Standard injury/incident numbers and frequency rates will be recorded for Lost Time Injury,
Restricted workday case, Medical Treatment Injury, First Aid Injury, Reported Incidents and all
Incident frequency rate.

The HSE Assessor/Officer will report the statistical indicators on a monthly basis.

17. ACCIDENT REPORTING & ANALYSIS

The investigation and reporting of incidents and accidents occurring as a result of the PT ASL
Shipyard Indonesia operations and within an area of PT ASL Shipyard Indonesia responsibility will
be conducted in accordance with PT ASL Shipyard Indonesia investigation procedures.

The primary purpose of incident investigation and analysis will be to provide a mean of
improving HSE performance on the project and thereby reducing both direct and indirect losses
and enhancing HSE of each working environment and protection of the environment. The
procedure outlined below aims to provide effective reporting and investigation of all accidents,
incidents and near misses arising in the course of PT ASL Shipyard Indonesia related operations in
order to:

• Establish the facts, circumstances and root causes of incidents,


• Communicate to appropriate personnel the results of investigations and any remedial
measures required,
• Identify future preventative measures or areas of improvement which may reduce losses in
respect of those operations,
• Comply with any legal reporting requirements and to meet contractual obligations to
COMPANY.

The notification and reporting of all Incidents / accidents is also presented in the Incident
notification table reference See APPENDIX G

In any case of incident / accident, PT ASL Shipyard Indonesia shall immediately notify to
COMPANY Superintendent via email and submit a preliminary written Investigation report within
24 hours of the incident/accident. Full detail report will be submitted within 14 days.

PT ASL Shipyard Indonesia will provide injured parties with adequate medical care and will
perform the appropriate level of case management (i.e. Initial medical treatment and follow up
surveillance).

“incident” also includes “near miss”.

FIRST AID CASES An accident, which requires medical treatment, and subsequent
observation of minor scratches, cut, burn, splinter and so forth

18. AUDIT AND REVIEW

HSE audits will be undertaken in compliance with the HSE plan and Project procedures. A
HSE audit schedule will be prepared and maintained by the QHSE Manager or Asst. HSE
Manager. During Project life PT ASL Shipyard Indonesia will carry out Audit one-time at Worksite.

18.1 AUDIT TEAM

The audit team will typically consist of personnel from the following:

18.2 FOLLOW UP AND MONITORING

A complete audit report will be made to the Director on completion of the audit. Areas of

19.2 PROCEDURE

19.2.1 PROJECT/VESSEL NAME Master/ Captain and/ or COMPANY Superintendent will liaise
with Project Manager during the VSCC meeting for the proper disposal of any hazardous
waste on board the vessel.

19.2.2 Project Manager will designate authorized contractor for the disposal of hazardous
waste.

19.2.3 The shipyard will provide bins for PROJECT/VESSEL NAME for the domestic waste and
contaminated items such as oily rags, hand gloves, etc. must put inside the plastic bag
before putting in the bins on board the vessel.

19.2.4 The shipyard will remove once it full and replace empty bins until the project was done.

19.2.5 Sub-contractors will provide their own bins for their own waste such (rubbish and scrap)
during the period of the project.

19.2.6 A daily housekeeping will be done by all the sub-contractors and PROJECT/VESSEL NAME
sub-contractors during the period of the project.

20. WASTE MANAGEMENT PLAN

The purpose of this plan is to ensure that domestic refuse, solid waste, used oil, sludge,
hazardous and toxic waste and all waste generated from the PT ASL Shipyard Indonesia are
properly handled and disposed in order to minimize impact to the environment.

The plan covers the management and disposal of these wastes that are too carried out by

20.1 PROCEDURE

20.1.1 RESPONSIBILITY

• For construction of project or vessel supervised by Project Manager, New Building and
Facility it is responsibility of the contractor to dispose of all waste generated from
construction site to the authorized dumping ground. The disposal of the waste must be in
accordance to the local requirement also. Project Manager shall ensure that these are
properly carried out by contractor.
• For all disposal of hazardous wastes and toxic waste, all records of disposal shall be
acknowledged by Project Manager and Logistic Manager.

20.1.2 MANAGEMENT DISPOSAL OF SOLID WASTE

• All domestic refuse to dispose of by the contractors appointed to the project shall be
dispose all wastes to designated dumping ground. All vehicles used by contractors for
dumping of domestic refuse should bring directly to the dumping ground. The
Department/Section Head shall responsible to monitor the dumping waste by their
contractors and shall keep a record of the monitoring.
• For disposal of solid waste generated in the PROJECT/VESSEL NAME, the contractor
engaged by respective Department / Section of ASL shall dispose of all waste to the
authorized dumping ground. See APPENDIX K

20.1.3 MANAGEMENT DISPOSAL OF HAZARDOUS WASTE:

• Used lubricating oil


• Chemical cleaning contamination
• Used blasting materials (copper slags, garnet, steel shot)
• Contaminated rags
• Sludge Oil
• Chemical Paint
• Fats
• Asbestos
• Used battery, printer cartridge
• Expired medicine

a) Hazardous and Toxic waste of PT ASL Shipyard Indonesia shall be collected in store or
designated location for temporary waste storage.
b) Used copper slags should be collected into jumbo bags and used oil will be collected
into drums or portable tanks.
c) Management will appoint or contact approved contractor to dispose the waste within
90 days storage.
d) The contractor shall provide a Manifest Document of the waste loaded.
e) After completion of work, manifest document should be acknowledge by Director,
Store man shall collect the manifest for documentation. See APPENDIX L.

20.1.4 PROCESS MEASUREMENT

• Collected used oiled / sludge oil into drum and used copper slags into jumbo bags
every day.
• Disposed the waste from legal disposal area every 90 days.
• Collected organic rubbish canteen into closed bin using plastic bag every day.
• Send domestic and organic waste to dumping area, do not wait the bin smell and
maggot indicated twice a day.

20.1.5 RECORDS

• Hazardous Material Waste Inventory to keep records by Store Supervisor for 2 years.
• Solid / Domestic Rubbish Inventory to keep records by Store Supervisor for 2 years.
• KLH B3 Manifest Document to keep records by Store Supervisor for 30 years

21. JOURNEY MANAGEMENT PLAN


21.1 Individual Traveller

Each traveller is responsible for initiating their preliminary travel requirements, beginning
with obtaining an approved Travel Authorization Form prior to making travel arrangements.

NOTE: Travellers are responsible for having copies of their passport, visa and work permit.

21.2 Procedures for Project Personnel’s related travel to Batam :

• Expatriate must ensure passport is valid with at least 6 months remaining before
expiration and with at least two blank pages.
• All traveler must be has been appointment with PT ASL Shipyard Indonesia Customer
service department and will directly pickup use PT ASL Shipyard Indonesia
transportation Car from Ferry port or Airport.
• All transportation vehicles driver can recognized with PT ASL Shipyard Indonesia
uniform brown color with identification ASL Marine logos on the front and behind side.
• The PT ASL Shipyard Indonesia vehicles driver will take you to PT ASL Shipyard
Indonesia
