LOAN AGREEMENT

I, SAMUEL DELA ROCA LEOMO of legal age and a permanent resident of 264 Montillano St.
Alabang Muntinlupa City , voluntarily entered in a LOAN AGREEMENT with MYTRON
MAUSEMENT CENTER PROVIDENT FUND, hereby represented by MR. JULES G. AJUSTO of legal
age and a resident of Alabang, Muntinlupa City for which I barrowed the amount of THIRTY
THOUSAND PESOS ( P 30,000.00) with the following agreements :

1. The said loan would have an interest of 10% per month

2. The said loan has a duration of one ( 1) month. From February 22 to March 22,
2017. With a total of P 33,000.00 inclusive of the 10% interest for one month.
3. That in the event of non-compliance on my part to settle the said amount agreed
together with the accumulated interest I will be legally liable to settle my
obligation in any court of law in the Philippines.
4. Failure to settle as agreed upon with reference to the duration date and the
amount agreed upon a penalty of 2% per day will be added on the principal loan of
P 30,000.00.

This agreement was entered between Mytron Provident Fund and me in the presence of the
witnesses whose signature are fixed herewith. And I hereby attest on my full and willfully
adherence to all agreements and provisions herewith.
