CERTIFICATE OF ACCOMPLISHMENT AND SERVICE

This is to certify that I have rendered full service as prescribed by Executive Order No.
292 and the Civil Service and Regulations performed the following task as a Driver
Assigned in Admin/Commission on Audit, for the period May 16-31, 2021.

This is to certify that the following tasks were accomplished by the undersigned from
May 16-31, 2021:

1. Drive passengers from the office and vice versa safely.

2. Maintain cleanliness of service motor vehicle.
3. Keep service vehicle in good condition, which include checking of the brakes,
fluid, engine oil, radiator coolant, under chassis parts, etc.
4. Perform simple trouble shooting of the vehicle.
5. Prepare trip tickets.
6. Photocopy of documents.
7. File documents.
8. Perform other duties that may be assigned by superiors.
