    DECLARATION

    We the undersigned ……. wife of / son of ……. aged 50 years residing at


    about do hereby solomnly affirm and state on oath as follows:-

    Whereas late Sri/smt ........ was a member of The Bangalore City Co-
    operative Bank Ltd., Pampamahakavi Road, Bangalore-18, holding Shares
    expired on ……… leaving behind her the mentioned persons as the only
    heirs entitled to claim the shares & DRF/title deeds relating to the house
    property the security of which he/she had raised loans which was
    discharged on.

    On our application dated......to the General manager, The Bangalore City


    Co-operative Bank Ltd., Bangalore-18, for the return of the shares & DRF /
    title deeds of the above property, the Bank insisted upon the execution of a
    declaration by us before the return of the title deeds.

    Further we authorise Smt. to receive the Shares & DRF/documents of title


    of the above property on our behalf. We declare that in the event of any loss
    on this account to the Bank. We shall indemnity such loss that may be
    incurred by the Bank.
