Detail of incident/accident: On October 1, 2022, at approximately 9:00 am

Kennel Harvey and I was changing a broad base receptacle in the living room at
Kennel home 6 cock burn lane Kingston 8. While Kennel was standing on the iron
chair changing receptacle, he suddenly lost his balance and fell, injuring his right
arm. I quickly ran for the first aid kit, retrieved an ice pack and piece of cloth, and
placed it on the arm that seems broken. I called one of his family members who
helped me to take him to the nearby doctor’s office. The doctor then offered further
treatment and then transferred him nearest hospital.

The accident was a result of using the wrong equipment which was the iron chair.
To prevent an accident such as this from reoccurring I recommend that the correct
equipment be used in this case a ladder.
