Tanzanian and a resident of Mwanza do hereby swear and state as follows:

1. That all two names of RAJABU SAID SHABANI and RAJABU SAID MFANGA are my
official names as they appear in my national Identification Card with Number 1999-
02233-32230-000129 and my Academic Certificates respectively.

2. That, in the course of dealings I have been using all the two names of RAJABU SAID
