CHANGE OF NAME BY DEED POLL

(Made under Registration of Documents Act Cap 112 R.E. 2002)

This change of name deed made this 21st day of MARCH, 2022, by me, the
undersigned MATIAS LUJELI GELIVAS of MOROGORO, Tanzanian citizen and it is here
by declared as follows:-

1. That, I absolutely and entirely renounce, relinquish and abandon the use of my
said former name of MATHIAS LUJERI MAYANGUNA as it appears wrongly in
my NSSF CARD and assume, adopt and to ermine to take and use from the
date hereof, the name of in substitution for my former name of MATHIAS
LUJERI MAYANGUNA
2. That, I shall at all times hereafter in all records, deeds, documents and other
writing and in all actions and proceedings as well as in all dealings and
transactions and on all occasions whatsoever use and subscribe the said name
of MATIAS LUJELI GELIVAS as my name in substitution for my former name of
MATHIAS LUJERI MAYANGUNA so relinquished as aforesaid to the intent that I
may hereafter be called and known or distinguished not by the former name of
MATHIAS LUJERI MAYANGUNA but by the name of MATIAS LUJELI GELIVAS
3. That, I acquired the said name of MATIAS LUJELI GELIVAS instead MATHIAS
LUJERI MAYANGUNA as later name was wrongly written on my I authorize and
require all persons at all times to designate describe and address me by the
adopted name of MATIAS LUJELI GELIVAS
4. In Witness Whereof I have hereunto subscribed my adopted and substituted
names of MATHIAS LUJERI MAYANGUNA instead of MATIAS LUJELI GELIVAS.
5. That, I made this declaration without an intention to defraud or to deceive
any one and anything.
