residence at Old Buncag, Brgy. Mandaragat, Puerto Princesa City Palawan do
hereby constitutes and appoints CECILDA CANJA likewise of legal age, Filipino
with residence at 27 Kakawati Street, Western Bicutan, Taguig City to be my true
and lawful attorney-in-fact, for me and in my name, place and stead, to do perform
the following acts, to wit:

1. To inquire, request, get, represent, process, receive the documents under

Philhealth, PAG-IBIG and SSS (Social Security System).
2. To sign, execute and deliver any all documents related thereto and perform
other acts necessary to fully accomplish the above purposes.
