1. THAT I am an adult female citizen of the Republic of Kenya hence competent to
swear this statutory declaration.
2. THAT I was born on 16th December, 1994 in accordance with my Certificate of
Birth No. 882703 and issued to me on 7 th March, 2013 and Entry No.
06600503/13. Attached hereto is a copy of my Certificate of Birth and a letter of
verification of birth certificate from the Ministry of Interior Coordination of
National Government Ref No: CRS/LIM/VER/VOL2/75 dated 31st January, 2023.
3. THAT my National Identity Card No. 32615882 states that I was born on 16th
December, 2015. Attached hereto is a copy of my National Identity Card
4. THAT I swear this affidavit confirming that the said Birth Certificate and National
Identity Card mentioned above belong to the same person being me
5. THAT I swear this affidavit conscientiously, and the same being true to the
best of my knowledge and belief.
