That I am a citizen of the Republic of Kenya and holder of Identity Card No.
0721129.

That the said Identity Card bears the names JAMES NJEHU MBOCHI
NJEHU.

That I am also the holder of a Kenyan passport Number A329337 which bears
the names JAMES NJEHU MBOCHI.

That the names contained in my said passport omit the surname NJEHU
which is my family name.

That I would like the said surname NJEHU included as per my identity card.

That I swear this affidavit in declaration that I would like to have my family
name Njehu added as a surname in my passport.

That what is deponed herein is true to the best of my knowledge, information

and belief.
