WAIVER

I, ___________________________________, resident of
_____________________________________________,understand the conditions for my
participation in the 4-H Regional Youth Camp on September 13-15, 2022 at DORSU,
Mati City, Davao Oriental. The conditions are as follows:

 The participant is not younger than 18 years old;

 It is understood that they are free to participate in the activities. There will
be no entrance fee to participate, however, they will be in-charge of their
own expenses such as accommodation, food, and transportation to and from
the venue;
 The ATI-RTC XI will not provide reimbursements or refund to the additional
participants;
 Their participation is completely voluntary.
