Consent agreement for Interview with Minors

Dear Parents,

Thank you very much for accepting the interview with The Centre for Child Rights and Business (“The
Centre”).

The Centre is a social enterprise that supports companies to deliver improvements within their supply
chains that benefit workers, families, and children.

We have both agreed that it is currently not the right time for your child to be involved in work in China,
and the best option is for your child to continue with education, which will be beneficial to his/her
future in the long run. Thus, we would like to understand the situation to develop an educational
support programme (the Programme) with your and our client’s support (Our client is also a client of the
factory/production site your child was working at).

We hereby would like to seek your consent in allowing The Centre to collect the necessary data. The
necessary data include name and images of child and legal guardian, ID copy of child, contact
information, education background and working experience of child, general family financial situation,
etc. This data will be used to develop a proposal for educational support programme, and will allow us
to continue supporting you and your child. All personal data will be handled confidentially and not
shared with anyone not directly involved in the process, and any sharing would always be done only
with your explicit consent. We will not share any personal data when reporting back to the factory/
production site and their clients. Any reports they will receive will be fully anonymized. All personal
data will be destroyed latest 2 years after completion of the educational support programme. We will
strictly protect you and your child’s privacy and act in full accordance with China’s Personal Information
Protection Law.

By signing this form, you as a legal guardian of the child agree to the above terms. If you have any

Please feel free to contact us when you need or when you feel that your rights according to China’s Laws
