    AFFIDAVIT OF ENGINE

    I, RICARDO PO, authorized representative of LEYTE UNITED TRADING, of


    legal age, married and resident of 104 P. Burgos St. Tacloban City, after being duly
    sworn to according to the law do hereby depose and say:

    That I am the owner of a vehicle and I caused the changing of the engine
    of a motor vehicle more particularly described as follows:

    Make: FUSO
    Type: TRACTOR HEAD
    Motor No: 8M22-S42951S
    Chassis No: FV513H-500315
    Plate No: RHL-680
    File Number: 0386-179632

    That the Change engine of the above vehicle will be changed from 8M22-
    S42951S to 8M21-010047. That the changing of the engine will be effective upon
    one approval of the application by the lawful authority.

    That I am executing this affidavit in order to attest to the truth of the


    foregoing statements and to other legal intents and purposes.
