Per the records of the company, a Return to Work Order was issued to you on two separate
occasions, September 22, 2022 and September 28, 2022, in both instances you had refused and
have continuously refused to return to work.

On October 13, 2022, the Company issued a Notice to Explain to you, to give you an opportunity
to reply in writing why no disciplinary actions should be taken against you for your continued
absence without leave. Unfortunately, you also failed to submit your written explanation.

Hence, the Company shall resolve the issue based on the documents available. From the
foregoing, it readily appears that you have no intention to return to work and have effectively
absconded and abandoned your work.

Thus, you are hereby DISMISSED from your employment.
