1. Respectfully request that serology examination be conducted on the
accompanying specimen to determine whether there is a human blood of the same
type with the blood of the victim Edwardo Mercado y Canceran.

2. Nature of Case: Stabbing Incident

3. Victim:
EDWARDO MERCADO y CANCERAN, male, 55 years old,
DOB: September 4, 1967), single, jobless and residing at No. 4367
L Bernardino Street, Gen T De Leon, Valenzuela City. (Deceased)

4. Suspects:
ERWIN ALVERO BAGTAS, male, 33 years old, (DOB: August 25,
1987), jobless, and residing at No. 4373 L Bernardino Street, Gen T De
Leon, Valenzuela City. (Under Custody)

5. TDPO:
At around 1:30 AM of December 26, 2020 along L Bernardino Street,
Gen T De Leon, Valenzuela City.

6. Specimen submitted:

a). One (1) piece of kitchen knife with 12 inches long marked
as “RAR”
b). One (1) piece of white T-shirt with design marked as “RAR-1”
c). One (1) piece of color black and white beach short with design
marked as “RAR—3; and
d). One (1) piece of white face towel marked as “RAR-4”.
(All are owned by the suspect Bagtas)

Na, ako ay kasalukuyang nakatalaga bilang imbestigador ng Station

Investigation Unit ng Valenzuela City Police Station na may takdang oras sa
pagtratrabaho mula alas 7:00 ng gabi hanggang alas 7:00 ng umaga araw-araw na
kung minsan ay lumalabis pa sa aking tungkulin kung kinakailangan;

Na, mga alas 4:30 ng umaga petsa ika-26 ng Disyembre 2020 habang ako
kasama ang mga kapwa ko imbestigador ay tumutupad at gumaganap ng aming
opisyal na tungkulin ay mangyaring kami ay makatanggap ng tawag na mula kay
S/G Mirando ng Valenzuela Medical Center na kung saan ay nag-iimporma na itong
si Edwardo Mercado y Canceran na taga No. 4367 L Bernardino Street, Gen T De
Leon, Valenzuela City ay sinugod lulan ng tricycle patrol ng Brgy Gen T De Leon na
minamaneho ni B/T Elmer Arcega dahil ito ay sinaksak;

Na, dahil sa natanggap namin impormasyo ay walang inaksayang oras ay

agad ako kasama si PSSg Mark Gil Bolanio ay nagtungo sa nasabing ospital para
alamin ang tunay na pangyayari lulan ng mobile 432;

Na, doon ay positibo namin nalaman na itong si Edwardo Mercado y

Canceran ay patay base sa kanyang doktor na si Dr Carl Linni Malibiran ay
dineklarang Dead on Arrival mga bandang alas 2:05 ng umaga petsa ika-26 ng
Disyembre 2020;

Na, matapos naman malaman ay agad kami nagtungo sa lugar kung saan ay
nangyari ang krimen na matatagpuan sa kahabaan ng L Bernardino Street, Gen T
De Leon, Valenzuela City para sa masusing pag-iimbestiga;

Na, pagdating namin doon ay aking nakausap itong si B/T Raul Arguel na
unang rumesponde sa nasabing lugar at kung saan din ay kanyang itinuro sa akin
na kanyang nakita ang isang (1) kitchen knife na kulay dilaw ang hawakan na
nakasiksik sa gilid ng timba kasama nito ang damit ang T-shirt, short at face towel na
nakahalo sa mga labahin na di-umano gamit ng tumakas na suspek matapos ang
mangyari ang krimen;

Na, matapos maproseso ang lahat ng narekuber na ebedinsya ay akin din

nalakap ang buong pagkakakilanlan ng nasabing suspek na si Erwin Alvero Bagtas
na taga No. 4373 L Bernardino Street, Gen T De Leon, Valenzuela City;
 Na, dahil sa narekuber na mga ebedinsya ay akin naman dinala sa tanggapan
SOCO-NPD upang ipasuri ang mga dugong naiwan sa mga narekuber na
ebedinsiya;

Na, bilang patunay ang lahat ng aking nabanggit na salaysay ay aking

lalagdaan sa ibabaw ng aking pangalan ngayon ika-28 ng Disyember 2020 dito sa
lungsod ng Valenzuela.
