DISCHARGE PLANNING

MEDICATION
In Hospital
 Hyoscine IV now and then every 12 hours
 Paracetamol 300mg IV every 4 hours if T>37.8C
 Ranitidine 50mg IV every 8 hours
At Home
 Ciprofloxacin 500mg tablet. 1tablet 2x a day for 5 days
 Metronidazole 400mg capsule. 1 capsule 3x a day for 1 week
 Omeprazole 40mg capsule. 1 capsule once a day for 1week
 Losartan 50mg tablet 1 tab once a day for 1 month
 Multivitamins capsule 1 capsule once a day for 1 week

ENVIRONMENT
 Always kept the environment clean and safe
 Keeping the kitchen and bathroom clean

TREATMENT
In Hospital
 Restrict solid food intake
To allow for bowel rest and reduce intestinal workload
 Advise patient to increase fluid intake to avoid dehydration
 Administer anti diarrheal medication as prescribed
At Home
 Advise patient to comply medications as prescribed by the physician.
 Drink plenty of fluid and rest at home

HEALTH TEACHINGS
In Hospital
 Do not take any medication unless directed by the physician
At Home
 Wash hands thoroughly before handling foods and after handling the bathroom
 Advise to wash and cooked foods properly before eating

OBSERVATION / OPD
 Advise patient to report any signs and symptoms of gastrointestinal disturbances like
nausea and vomiting and reoccurrence of pain
 Follow up at OPD after 1 week to ensure the continuing management and treatment.

DIET
In Hospital
 Advise to eat small amount of mineral-rich food
 Avoid foods and drinks that are high in dairy and sugar
 Advise patient to have small frequent meal
 Advise patient to avoid spicy foods because it may cause GI irritation
At Home
 Advise patient to eat her meal on time and avoid highly seasoned foods for a few days
 Eat fruits like banana and apple
 Increase oral fluid intake and return to normal diet, as tolerated

SPIRITUAL
 Advise patient to have strong faith in God and keep praying.
