Dear Kuya Eagle Anthony,

I, Leo Capuyayan Victoriano, currently residing at Blk 50 Lot7 Usnai Comp.Brgy 173 North
caloocan City 1420, would like to take this opportunity to declare my intention to be a Member of
your esteemed club. I am willing to undergo and pass through the following steps called the five (5) I’s
of accepting new members, or the 5 I’s, which are follows to wit;

3. Initiation
4. Incubation
5. Induction
