Junior Basketball Tournament Waiver

This waiver form requires the parent or guardian of a minor participating in a 3x3 Basketball Tournament Junior Division to agree to abide by all rules of the tournament. It also recognizes the risk of physical injury from participating in tournament activities and releases the tournament and its staff from any liability for injuries sustained on the tournament property or during activities. The parent or guardian must sign the form to acknowledge their agreement.
