Injection. Religrast 300mcg subcutaneous (Deltoid region only ) once a day x 2 day on 06/01/2023 ( First dose of
Grafeel to be taken 24 hours after completing of chemotherapy )

Tablet. Crocin 500milligram half hour prior to Injection. Filgrastim,and sos in case of fever/bodyache ( up to three
times a day with a gap of atleast 8 hours between 2 doses )

Tablet. Emeset 4mg 1 tab After meal ( Breakfast, Lunch and Dinner ) for 4 days

Tablet PANTOCID [PANTOPRAZOLE ] 40 mg once daily [empty stomach morning] for 5 days

Tab. Crocin 500 mg three times a day for pain

Syrup Neogadine 15 ml twice a d ay to continue

Tablet ENDACE 80 mg twice a day to continue

Tablet SUPRADYN [ MULTI VITAMINS ] 1 tab once daily (After dinner) to continue [10:00pm]

MUCOBENZ mouth wash four times daily (2 teaspoons of MUCOBENZ rinse and gargle for 30 seconds, preferably
after meals)

Plenty of oral fluids

Tablet LIBRAX [ CHLORDIAZEPOXIDE + CLIDINIUM BROMIDE ] 1 tablet as & when required for pain abdomen (you
may take upto 3 or 4 times a day with a gap of at least 6 hours between 2 doses. take the medicine preferably before
food). [please do not drive a motor vehicle or operate a machine while you are on treatment with this medication]

Injection PERINORM [ METOCLOPRAMIDE ] 1 ampoule Intravenous [i/v] as & when required for vomiting (you may
take upto 3 or 4 times a day with a gap of at least 4-6 hours between 2 doses. however, if vomiting dose not subside,
please report back to emergency department at Medanta)

Syp. LOOZ [LACTULOSE] 15 ml at bedtime in case of constipation. (Syp. LOOZ [LACTULOSE] 15 ml can be taken
upto three times a day according to individual need. Please do not take if you are having loose stools.)

Please continue your ANTIDIABETIC & ANTIHYPERTENSIVE treatment under supervision of Cardiologist /
Endocrinologist, if any

You are requested to report to sample collection room at upper ground floor 4-5 hrs before OPD appointment time.

When you visit the hospital for chemotherapy, you will be assessed in the out-patient department by one of the team
members to determine your fitness for chemotherapy, which generally depends on the physical parameters and blood
reports. Therefore, it is advisable to have an appointment booked in advance with the front desk in the out-patient
department whenever you plan to visit the hospital for a planned procedure like chemotherapy.

Administration of chemotherapy is subject to physical fitness of an individual and also satisfactory blood reports. In
order to save time, you are encouraged to give the blood sample in the phlebotomy room (blood sampling room),
located in the ground floor, as soon as you arrive in the hospital.

You are requested to report to OPD with all relevant reports as advised by the treating team
