UNDERTAKING

The undersigned, , Inc., (hereinafter referred to as the “Company”) hereby authorizes the issuance of
UnionBank Corporate Credit Card(s) (the”Cards”) for its employees listed hereinbelow (the “Cardholders”). The
Company undertakes to pay the UnionBank of the Philippines (UBP) all the obligations, charges and liabilities
incurred through the use of the issued Cards in accordance with the terms and conditions governing the
issuance and use of the Cards as printed at the back of the Company Enrollment Form or Card Carrier.
Notwithstanding any change or revision in the said terms and conditions, this undertaking shall continue to bind
the Company until such obligations, charges, and liabilities shall have been fully paid and satisfied.
