I am writing to request a new copy of the evaluation sheet for Bachelor of Secondary
Education - Major in English that was provided to me by Mrs. Helen T. Rumpon.
Unfortunately, I have misplaced the original evaluation sheet and am unable to locate it.

I understand that the loss of the evaluation sheet is solely my responsibility, and I apologize
for any inconvenience this may cause. As a result, I would be grateful if you could provide
me with a new copy of the evaluation sheet so that I can have an accurate record of my
academic performance.

Please let me know if there are any applicable fees or charges associated with obtaining a
new evaluation sheet.
