This Certificate of Successfully Completed READY FOR SERVICE is granted by PT. Eka Mas Republik in
relation to the Certification Package for completion of READY FOR SERVICE No.
BKS002465/RFS/02/2023/7400003716 dated 13 February, 2023 submitted by Vendor pursuant to
Purchase Order No. 7400003716

Vendor agrees and fully understands that the occurrence of any Successfully Completed Acceptance of
READY FOR SERVICE shall not otherwise affect in anyway the obligation of Vendor to complete all
obligations of Vendor as set forth in the Contract Documents.

Within the Warranty Period and/or Extended Warranty Period(s), Vendor undertakes, at its own expenses, all
and any responsibility on errors, Defects and Deficiencies, or omissions discovered in respect of any
Equipment, Services and Functions within the Network addressed herein.

This Certificate is issued by PT. Eka Mas Republik and granted to ADYAWINSA ATE pursuant to Section
17.8 of the Master Agreement.

All and any capitalized terms used in this Certificate shall have the meanings provided in Exhibit A to the
Master Agreement.
