1. The approval of the Greater Amman Municipality or the municipalities in the
governorates. 2. The approval of the health directorates. 3. Civil Defense approval. 4. An electric elevator must be available in the building if the center is within the floors of the building. 5. The provision of service shall be limited to a technician for males and a technician for females. 6. That the center be supervised by a specialist doctor. 7. A certified copy of the doctor’s practice of the profession. 8. A letter of non-objection from the Medical Association. 9. A lease or title deed in the name of the applicant doctor. 10. Organizational site planner. 11. Submit a summons requesting a clinic license. 12. Disclosure by the committee for the approval or disapproval of the place designated for the clinic. 13. A contract with a company for the disposal and destruction of medical waste. 14. Required fees: A fees of (50) JD.
