OCCUPATIONAL THERAPY EVALUATION

Name : Retaj

Date of Birth : 1/9/2008

Date of Evaluation : 30/10/2022

Age : 14 years and 2 months.

EVALUATION METHODS
The Beery-Buktenica Developmental Test of Visual-Motor Integration (VMI)

HISTORY

Background Information/Reason for Referral

Retaj is a 14-year-old student studying in the ninth grade at her school. She is the youngest of her brothers and has two

older sisters and two brothers as well. Her school teacher noticed that she is slow in transferring information from the

board and that she does not complete her assignments assigned to her in the class in the time allotted to her, in addition

to that she shocks her classmates when walking in the yard.

The teacher had attended a lecture on the problems that children face during the developmental stage, so she suggested

that the parents take Ritaj to an occupational therapist to check if she suffers from strikes and needs intervention.
The teacher mentioned that the retaj's writing performance is good on her notebook, but she finds it difficult to read

from the board and copy to the notebook.

Medical History

Retaj’s pre natal and birth history was within normal limits.

Developmental History

Early developmental milestones were reported to be within age appropriate ranges. No concerns were expressed

through preschool and kindergarten years.

TEST BEHAVIORS AND SCORES

Retaj was seen over one evaluation sessions. . In the session she participated for about 30 minutes and she was
able to engage in all tasks.

She was excited to try the new activity and found it fun at first, and at the end of each test she struggled with a
solution. All tasks were completed in a timely manner without any delay. The VMI work took 3 minutes, the visual
perception took two and a half minutes, and the motor coordination took 4.20 seconds.

The following results appear to be an accurate assessment of Retaj’s current abilities considered in this
evaluation

Developmental Test of Visual Motor Coordination - VMI

This standardized test yields a standard score and range of performance concerning a student’s ability in the

reproduction of progressively more difficult geometric forms copied from models. Standard scores have a mean of

100 with a standard deviation of 15. The student’s abilities for processing multiple subskills such as pencil grasp

and control, coordinated hand use, desk posture, spatial and directional awareness, sequential movement patterns,
accuracy in closure, and attention and organization of the task are also observed.

The accurate and efficient processing of these critical subskills is a prerequisite for the development of adequate

handwriting skills. Subtests consist of visual perception and motor coordination.

Retaj scored as follows:

STANDARD SCORE PERFORMANCE

VISUAL-MOTOR INTEGRATION 103 Average

VISUAL PERCEPTION 101 Average

MOTOR COORDINATION 101 Average

Discussion

Retaj’s standard scores do not show any significant delays. She is within average.

She did not encounter any difficulty during her performance of the task, following orders exactly as they were

transmitted to her. She uses her dominant right hand to hold the pen and her left to hold the paper. She was

bringing her body closer without feeling it on the paper, but with verbal help, she would return her body as it was.

Retaj didn’t applied heavy pressure on the pencil while administering the standardized test.

She seems to have problems in vision.

Handwriting Sample

Retaj wrote a complete text in Arabic that was verbally transmitted to her, and copied a small paragraph

in English from a piece of paper placed on the table next to her without any difficulty and in a clear

handwriting. However, she was unable to transfer the Arabic or English text, or even to copy numbers

and symbols from the blackboard, which is 6 meters away from her table.

INFORMAL MOTOR OBSERVATIONS

Gross/ Sensorimotor

Retaj seemed normal and typical and she have the ability to stabilize the trunk and neck to enable
coordination of other limbs. And she has the ability to maintain her body position whether she is static (sitting),
moving or rotational in her place. Accurate registration, interpretation and response to sensory stimulation in the
environment and her body. When asked her to imitate certain postures against gravity, retaj was able to hold his body
against gravity without any difficulty. In general, her gross motor skills are age appropriate.

Fine/ Sensorimotor
During the session, I observed that she held the pen in a correct manner (tripod). She held the pen with her

dominant right hand and fixed the page with her left hand (bilateral integration). She moved the hand and the

line in coordination with her gaze (eye hand coordination).

Retaj also showed appropriate finger and hand strength.

SUMMARY AND RECOMMENDATIONS

Retaj is a 14-year-old girl studying in the ninth grade. Her teachers noticed that she was suffering from difficulties in

the classroom and that she could not copy and transfer materials from the board. She sought the help of an

occupational therapist to evaluate Retaj's condition and determine whether she needed intensive evaluations or not.

After the VMI evaluation, the results showed that she is at the normal level and does not suffer from any problems

or delays compared to her classmates.

It seems that she suffers from visual problems and I suggest that she be shown to an ophthalmologist to examine her

and make sure that her vision is correct.
