WHEREAS, an Information has been filed in the Regional Trial Court, Branch 14,
Laoag City, charging the accused with the crime of Violation of _______________ as
amended, and was allowed by the Court to post a CASH BOND for his temporary
liberty in the amount of TEN THOUSAND PESOS ONLY (P10,000.00).

NOW THEREFORE, I, AAA, of ________________ as Bondswoman in the above-

entitled case, I hereby undertake that the accused will appear and answer the charge
against him in whatever Court he may be tried and will at all times hold ourselves
amenable to the orders and processes of the Court that his failure to appear at the trial,
without justification despite due notice shall be deemed an express waiver of his right to
be present on the date specified in the notice and in such case, the trial may proceed in
absentia and if convicted, they will appear for judgment and will be amenable and
available to the execution thereof and should they fail to perform any of these conditions
that we will pay the Republic of the Philippines the amount of TEN THOUSAND PESOS
ONLY (P10,000.00), in Philippine Currency, which deposited with the LAND BANK OF
THE PHILIPPINES, Laoag City Branch in the name of REGIONAL TRIAL COURT,
BRANCH 14 Savings Account No. ______________ as evidenced by the deposit slip
attached hereto.
