AFFIDAVIT OF UNDERTAKING

I, _________________, ___ years old, married, Filipino and a resident of

_____________________, after having been duly sworn to in accordance with
law, do hereby depose and state:

I am the bondswoman for _________________________ in the above-

captioned criminal case pending before this Honorable Court;

I have posted cash bond in the amount of Twelve Thousand Pesos

(Php12,000.00) for accused ____________________ evidenced by Official
Receipt No. ____________ dated _____________, for the provisional liberty of
accused ___________________________ and I undertake and agree to the
following conditions:

1. That this undertaking shall be effective upon approval by the court

and shall remain in force at all times, in all stages of the above-
captioned case and until its final determination, unless, the proper
court directs otherwise;

2. That I shall appear in court during the trial of the afore-mentioned

case and whenever required by the court;

3. That in case of my failure to appear at the trial without justification

despite due notice, my absence will be deemed an express of my right
to be present on the date specified in the notice, which will warrant
the court to proceed with the trial as if I am present;

4. That I shall surrender myself for execution of the final judgment.
