Interdisciplinary model of care is the model of a collective interdisciplinary teams moving

patient centered care across the continuum. It provides multi disciplines across different

specialties to work together as a team improving care to provide successful outcomes for

patients. This every growing type of style has helped us as nurses work as a synergistic team

verses individual silos. Many hospitals have instituted Multi Disciplinary Rounds team. The goal

was to help facilitate our patient discharges home as well as the discharge times that our

patients actually leave the hospital on the units. The main goal was to involve the providers,

case managers, nurses, ancillary departments involved in the patients care to direct the care

and flow provided to our patients. The expectation is the ultimately affect the length of stay for

our patients with an end result in improved efficient patient care while maintaining costs and

driving down the length of stay of our patients.
