2022 Electrical contact in turbine building

Description of the event

During maintenance work associated with the scheduled shutdown of the plant, while an employee of a subcontracting company
(mechanical supervisor) was moving around the pedestal of the turbine, he unintentionally contacts met live electrical wiring
(220V) and suffers an electric shock.

Injuries
• Slight discomfort in the right arm derived from the electrical contact.

Causes

• Incorrect application of a procedure/instruction (LOTTO not performed).


• Inadequate barrier/guard (energized electrical cable without adequate protection).
• Failure to identify risks (absence of signage, work procedure without adequate definition of scope).
• Inadequate supervision of the activity (injured employee was not included in the work permit).

✓ Refreshment training for staff on particularization of work permits and LOTO.
✓ Extraordinary meeting with a subcontractor company to analyze the event that occurred, and lessons learned.

Lessons learned

✓ Strict compliance with the five rules of electrical safety (disconnect power source, interlock/block to prevent possible
feedback, verify absence of voltage, ground and short-circuit possible voltage sources, as well as signal/protect work area),
the existence of specific work procedures as well as the proper application of the work permit/LOTO system are key aspects
to prevent the occurrence of this type of incident.
