I am Jonelle Grace A. Osma, a faculty from PSU-Coron Campus, writing this
letter to apologize for the omitted name in my Report of Ratings for the subject ED 12: Integrating Course in Education. My student, Mariel R. Vizcarra, wasn’t included in the Report of Ratings I have submitted but she is enrolled in my class, actively participating in every review session for this subject.
By the time of creating my report of ratings, I copy-pasted each name of students
from my list to its corresponding number on the page. I didn’t notice that Ms. Vizcarra’s name wasn’t included in the second page of the ROR that I created. However, she is on my class record and complied all the requirements needed for the subject. I would like to request for the rectification of the error done and I do apologize for my mistake. I can’t afford to let her lose this opportunity for her to graduate on time due to my mistake.
I am hoping for your kind consideration regarding this matter since Ms. Vizcarra is a graduating student.
