REPORT FIRE

Introduction
The Occupational Safety and Health Regulations (Notification of Accidents, Dangerous
Occurrences, Occupational Poisoning and Occupational Diseases) were gazetted in 2004 under
the Occupational Safety and Health Act 1994 (Act 514). Through this Regulation, the
Occupational Safety and Health Committee is requested to:

 To investigate complaints, reports or matters in the workplace that are deemed unsafe
or unhealthy to employers.

 Conduct studies on trends and statistics of accidents, occupational diseases and


poisonings, and hazardous occurrences, and then identify control measures.

 Review safety and health policies and measures taken in the workplace, and submit
recommendations to employers for review if necessary.

Objective
The objectives of this procedure are to:

 Explain the action steps during an accident in the drawing room.

 Investigate the cause and root cause of the accident and carry out welfare
responsibilities.

 Prevent from recurrence of the same accident.

 Meet the legal requirements of the Occupational Safety and Health Act 1994 (Act 514)
and regulations related to occupational safety and health

Scope
 This scope should be applied when preparing reports of complaints, accidents, near
misses, dangerous incidents, such as fires, occupational diseases and investigations in
the living room.

Responsibility

 All staff, contractors, students and visitors shall be responsible for reporting all
accidents, dangerous occurrences such as fires or occupational diseases that occur in
the drawing room.

 The Occupational Safety and Health Committee shall be responsible for implementing
procedures for reporting complaints, accidents, near misses, dangerous occurrences
such as fires, occupational diseases and investigations.

Reporting Procedures to the OSHE Unit:

 All accidents, dangerous incidents, occupational poisoning or occupational diseases that


occur in the living room whether injured or not, must be immediately reported to the
Head of Department / Head of Division / Head of Unit.

 The Head of Department / Head of Division / Head of Unit for each department must
ensure that every accident, dangerous incident, occupational poisoning or occupational
disease that occurs in their respective division is reported to the Occupational
Environment, Safety & Health (OSHA) Unit. A report must be made as soon as the
incident occurs in the living room.

 In addition, the Health Center must also report to the Occupational Environment, Safety
& Health (OSHA) Unit, if any victims are being treated by the Health Center.

 If there are staff, students or visitors dealing with the University who wish to make a
complaint related to occupational safety and health to the Occupational Environment,
Safety & Health (OSHA) Unit, the Complaint Form must be completed and sent to the
Occupational Environment, Safety & Health Unit (OSHA).
 Details of the accident or incident must be recorded in the form provided.

1. All fields in the form must be filled in honestly and sincerely. The information provided
will assist the University Occupational Safety and Health Committee to take
precautionary measures so that similar accidents do not recur.

2. The completed form must be submitted to the Occupational Environment, Safety &
Health (OSHA) Unit for further action.

 For cases of serious accidents, the accident area should not be disturbed, except to
prevent more serious accidents and facilitate the provision of assistance.
Conducting an Investigation

 Complaints or reports received by the Occupational Environment, Safety & Health


(OSHA) Unit will be submitted to the University Occupational Safety and Health
Committee.

 Members of the University Occupational Safety and Health Committee will investigate
the incident / complaint as soon as they receive the complaint or report and then action
and follow -up action will be taken.

 Members of the University’s Occupational Safety and Health Committee will also take
photographs and evidence at the scene for review.

 Information and evidence from accident victims, accident witnesses, Head of


Department / Head of Division / Head of Unit or the relevant will be taken and filled into
the Accident Report Form.

 The results of the investigation will then be reported and discussed in the next
University Occupational Safety and Health Committee Meeting for further action / as
information and further action by Members of the Occupational Safety and Health
Committee and parties from the divisions or units involved.

 Further reports will be sent to the Department of Safety and Health.

Here are the Fire Prevention Measures

 Flammable tools / materials are not brought into the room,


 Charity turns off the electrical switch when finished using it,
 Do not smoke in prohibited places,
 Do not make your own electrical wiring connections,
 The room must always be clean of rubbish,
 Do not abuse fire extinguishers,
 Take time to attend fire prevention talks / training, and
 Report to the responsible party if there is damage to electrical equipment / misuse
DURING A FIRE
Common instructions during a fire in the living room:

1. Try to put out the fire with the fire extinguisher available in the living room without
endangering yourself.
2. Leave the building via the nearest exit and Congratulations as soon as possible.

3. Do not be confused when leaving the building


4. Do not go to the toilet and so on
5. Do not stop in the middle of the road or turn back
6. pick up items or something left behind
7. Report to the gathering place and make sure your name is called during a direct call
Emergency lane.
