WITNESSETH

WHEREAS, WE, the abovenamed ARBs are the registered owners of agricultural land
covered by Collective Certificate of Land Ownership Award (CCLOA) pursuant to Section 5 of
R.A. No. 6657, as amended by Section 10 of R.A. 9700, embraced by OCT/TCT No. A-07323,
more particularly described as follows:

A parcel of land with Lot No. 86 , Cadastral No. ____, Survey Plan No. GSS-12-
000148,LRC Record No. ______ located at Sitio Mervan, Masiag, Bagumbayan, Sultan Kudarat,
containing a total area of THIRTY FOUR THOUSAND THIRTY EIGHT (34,038) square
meters, more or less.

Beginning at a point marked “1” on plan being N 42 degrees 14 minutes E 4,819.18


meters from BLLM No. 1, Pls-778 to corner 1;

Thence N 79 deg. 41 min W 333.77 m. to point 2;


Thence N 15 deg. 28 min E 97.61 m. to point 3;
Thence S 82 deg. 23 min E 316.76 m. to point 4;
Thence S 05 deg. 56 min W 112.52 m. to point of beginning;

WHEREAS, WE, the abovenamed ARBs and legal heirs of the deceased ARB, have
agreed to terminate the collective ownership and for this purpose, have cause the CCLOA be
parcelized and the subsequent issuance of Computerized Titles (c-Titles) per validation and
approval of DAR.

WHEREAS, WE, the abovenamed ARBs and legal heirs of the deceased ARB, have
agreed for a share in the parcelized agricultural land covered by CCLOA of an area not
exceeding three (3) hectares award limit to each ARB based on the following:
 ☐ Actual area of tillage
☐ Actual area of occupancy
☐ Equal area among ARBs
☐ Organized group of ARBs who opted to subdivide into sub-group

WHEREAS, WE have agreed to allocate the lots through raffle draw undertaken by the
MARPO/Designated DAR Personnel N/A , or by among the ARBs in
the presence of N/A BARC or representative of any civic organization
in the area held on N/A at N/A .

WHEREFORE, WE have agreed to the subdivision survey and commit ourselves as


ARBs of our respective lots and area allocated and on the corresponding c-Titles to be generated,
registered and issued therefrom by the Registry of Deeds (ROD).

IN WITNESS WHEREOF, WE hereunto affix our signatures corresponding to our names

this September 29, 2022 at Brgy.Masiag, Bagumbayan, Sultan Kudarat.
