FLOOR ROTATION

Things to do in our rotation:

1. Kindly fill out the health declaration form for the health monitoring sheet via
Google Form. Link: https://forms.gle/kvqVRYSJRK37zR3Q7. This should
be done before you report in the hospital. If you have signs and symptoms of
COVID-19 please do not report on duty but schedule an appointment with our
university physician for a consultation or at the ER Department of SWU Med
(See Appendix A for the picture).
2. Fill out the health monitoring sheet in the guard house (Please see Appendix
B for the picture).
3. Memorize the nurses’ prayer by heart. Link:

4. We will have our circle 30 minutes before the time (5:30 AM) in the area where
you are assigned.
5. Kindly bring a half cross-wise index card with your 2X2 picture on it.
Someone will collect it and arrange it alphabetically.

Bring all paraphernalia as what was oriented.

Add-on materials to bring in our RLE:

a) Basin (Small-medium)
b) 2 pieces of rug or damp cloth
c) NANDA (any edition will do)
d) Drug handbook
e) 1/2 size Manila paper with plastic cover in the other side (for you to place your
paraphernalia during circle)

*For the books, you may borrow or bring your own materials.

6. Make sure that somebody will load all the patients on the floor
7. Accomplish the Post-evaluation Tool by the end of our rotation.
8. Please bring a small tray (small-medium tray) per student for our medication.
9. Bring 5 pairs of clean gloves since it isn’t free.
10.Kindly review charting, plotting, and FDAR making. All these skills

 11.Please accomplish the post-evaluation tool by Saturday (April 15, 2023),
11:59 PM as seen in appendix E. This is worth 30 points.
12. For the clearance, I need to see the signature of your previous CI in the
clearance sheet before I will sign at the end of our rotation.

o Laboratory Results and Diagnostic Results
• 1 NCP
• 1 FDAR
• Drug Studies (Will Depend on Patient’s Number of Drugs)
• 1 Discharge Instructions

*All requirements should be in hand-written, place it in a long bond paper with 1 inch
margin on each side. Please provide a cover page. All requirements should be
submitted on the next day during the quality circle.

For tomorrow (April 11, 2023), Group A should prepare the following
requirements, this is the same with Group B (April 13, 2023):

1 NCP
1 FDAR
1 Drug Study
1 Discharge Planning

Submit it during our circle in alphabetical order together with the index card
(April 11, 2023, 5:30 AM - Group A and April 13, 2023, 5:30 AM – Group B ).

Circle will commence at 5:30 AM in 3rd floor terrace area. All paraphernalia
should be ready and everything should be placed inside the manila paper with the
plastic cover on top of the manila paper. Make sure you make a perfect circle.

Wear N95 Mask at all times. Not KN95 or any type of face mask except N95.
