risk of developing disabling or life threatening diseases

why we offer you the Healthy Heart benefit.
When FundsAtWork meets Multiply, your money
works even harder
Multiply offers incredible savings on a variety of brands,
services and products. FundsAtWork and Multiply
reward you financially for your physical wellness with
the Healthy Heart Score. This is another preventative
measure that brings you even more rewards.
Your Healthy Heart Score – the way to your
pocket
The Healthy Heart Score gives you an indication of how
healthy your heart is. In addition to being a pathway to
more rewards for FundsAtWork members, it can also
lead to steps being taken to improve your health.
How the benefit works

As a FundsAtWork member who has taken
Multiply Premier or Multiply Provider you must
register for the Healthy Heart benefit.

Complete your FREE Health Assessment.

The Health Assessment results (i.e. blood
pressure, cholesterol, BMI and blood glucose) are
then used to calculate a score, together with your
4 smoker’s status.
The Healthy Heart Score identifies which category
the health of your heart falls into: green, amber
or red.
5 Rewards are then calculated based on your score,
your Multiply product and Multiply level.
6 Rewards earned for your Healthy Heart Score can
be used to boost your FundsAtWork retirement
savings account, your Momentum HealthSaver
account or can even be converted into Pick n
Pay smart shopper points if you don’t have a
HealthSaver account. The Pick n Pay smart
shopper card must be linked to your Multiply
membership.

• Green: low risk of heart disease compared to peers in the same

age and gender group.
• Amber: moderate risk of heart disease compared to peers in the
same age and gender group.
• Red: high risk of heart disease compared to peers in the
same age and gender group.
You can check your Healthy Heart Score on the Multiply website.
If you do not have a score yet, you’ll find information on the site
explaining how to get it.

* The insurance premiums are capped at a maximum of R3 000 per month for the purpose of calculating the Healthy Heart benefit.
** The Fund is either the FundsAtWork Umbrella Pension or Provident Fund, depending on your membership.

If you have both a HealthSaver account and a smart shopper card, the rewards will be paid into your HealthSaver account and
not the smart shopper card.

If the benefit is paid as smart shopper points, it will be half of the amount that would be paid into your retirement savings
account or HealthSaver account.

How to register for the Healthy Heart benefit

1. Once you have registered on Momentum’s website (further information on how to do this is explained in the member’s Quick
Guide), you must go to www.momentum.co.za and login using your Username and ID. Then you must select the “view details”
button under the Employee Benefits section. Select “Healthy Hearts Benefit” under the “Rewards” menu and, if you have Multiply
Provider or Premier, you can activate the Healthy Heart benefit. You can also call the FundsAtWork client contact centre on
0860 65 75 85 for help with the registration.
2. Complete your annual Health Assessment with Multiply. You can do this at Dis-Chem or Clicks.
3. The Healthy Heart benefit will be allocated to you with each qualifying insurance premium paid.
4. A confirmation email and SMS will be sent to you after the activation of your benefit. An email and/or SMS will be sent to
notify you of payment into your retirement savings account, HealthSaver account or smart shopper card.

Healthy Heart benefit premiums

The Healthy Heart benefit does not cost you anything; you just have to pay your Multiply Provider or Premier monthly
membership fee. Over and above the Healthy Heart benefit, Multiply offers you much more. Visit www.momentum.co.za/
