Health and Safety Checklist for Students

Sinai Health (SH) is committed to providing a safe environment for all students. Under the Occupational Health
and Safety Act (OHSA) employers must provide instruction, information, policies and procedures to safely
conduct work. Employers must take every reasonable precaution to protect the health and safety of students.

How to use the checklist


1. Preceptors are required to provide a health and safety orientation on the student’s first day on-site using
this checklist as a guide.
2. If a section is not relevant to your particular placement, please enter N/A.
3. Contact Occupational Health and Safety (OHS) x3907 at Mount Sinai (MSH) or x2802 at Bridgepoint
(BAH) with any questions.
4. Student and preceptor sign the completed checklist, and the student uploads a scanned copy to their
NirvSystem account. If the student does not have a NirvSystem account, a scanned copy can be
emailed to safety.msh@sinaihealth.ca or safety.bah@sinaihealth.ca

Calling in sick o Responsibility of students with even one mild symptom to stay home and contact
OHS 416-586-4800 x7425 (MSH) or 416-461-8252 x2803 (BAH)
o Exchange contact information for student, preceptor, and back-up preceptor for
communication in the event of student or preceptor unexpected absence
Review the process for responding if a student is involved in a workplace incident:
o If first aid treatment is required, report to the OHS Clinic at MSH (60 Murray
Workplace Street, 1st floor) or BAH (G.057). Hours of operation: M-F, 7:30am-3:30pm
Incidents and o If OHS is closed or major medical attention is required, visit the Mount Sinai
Illnesses Emergency Department
o Students and their preceptors must complete an incident report (SAFER) for all
work-related incidents/illnesses/hazards
Review student responsibilities under the OHSA including:
Occupational
Health and o Compliance with all SH policies (e.g. Scented Products and Fragrances Policy)
Safety Act o Reporting hazards, equipment malfunctions, contraventions and incidents to
(OHSA) their preceptor
Roles and o Not engaging in behaviour that may endanger themselves or others

o Knowing and exercising their three OHSA rights (to know, participate, & refuse)
Personal Protective Equipment (PPE) such as gloves, gowns, or face shields may

Students must be made aware of:
o Information on potentially aggressive patient behaviours and the process to
request support to enable safe patient care
o Chemical and biological hazards (e.g. procedures for Blood Borne Pathogens,
hazardous drugs, anesthetic gases, etc.) and how to follow-up with exposures
Area-Specific o Hazards in the physical environment including slip, trip, and fall hazards,
Hazards and patient/material handling, etc.

o Location of WHMIS/SDS binder

o Location and use of eyewash stations
o Emergency contact information for the department & hospital
o Unit/building-specific evacuation procedures (if additional assistance will be
required during an evacuation, contact OHS to set up a personal plan)
o Student responsibility to seek out this information if also placed in other areas
Review and reiterate with your student:
o Universal Masking Policy: A procedure mask must be worn in all public and

At Sinai Health, students are permitted to participate in the direct care of COVID-19
positive patients if the preceptor deems the student has the required knowledge,
Learner Scope skills, judgement, and access to PPE to do so safely. However, some schools place
of Practice and restrictions on student scope of practice that are unique to that program.
COVID-19
o Discuss any restrictions to the student’s scope of practice, noting if there are any
patient populations or tasks to which the student cannot be assigned
